Title of the Job Learner – Doctor
Objectives  Provide first-class independent healthcare for the local community in a safe,
comfortable and welcoming environment
 To promote an excellent public image of Sir Takhtasinhji Hospital to all
Patient – their relatives, Beneficiaries and visitors.
 The action regarding the necessary treatment can be received quickly.
 Proactively ensuring that all health care needs of patients are met.
 To Impart Team mindset among Medical Students.(by Giving Round to
faculty once during term )
Goal  Clinical training (clinical clerkship) shall follow the learner-doctor method.
The goal of thelearner-doctor method is to provide the learners with
experience of longitudinal patient care,being a part of healthcare system,
and hands-on care of patients in outpatient and in-patientsetting.
 To Provide with experience in :
 Longitudinal patient care,
 Being part of the health care team
 Hands-on care of patients in outpatient and in-patient settings.
Structure  The first clinical posting in Phase II shall orient learners to the patient, their
roles and the specialty.
 The Learner shall function as a part of the health care team with the
following responsibilities.
1. Be a part of the unit’s out-patient services on admission days.
2. Remain with the admission unit until at least 6 pm except during
designated class hours.
3. Be assigned patients admitted during each admission day for whom he
will undertake responsibility, under the supervision of a senior resident
or faculty member. (one patient/Student, minimum three patient during
term)
4. Participate in the unit rounds on its admission day and will present the
assigned patients to the supervising physician.
5. Follow the assigned patient’s progress throughout the hospital stay until
discharge.
6. Participate, under supervision, in procedures, surgeries, deliveries etc.
of assigned patients.
7. Participate in unit rounds on at least one other day of the week
excluding the admission day.
8. Discuss ethical and other humanitarian issues during unit rounds
9. Attend all scheduled classes and educational activities.
10. Document his observations in a prescribed log book/case record.
General  During 3rd Professional Part-1, there will be clinical postings for 4 weeks
Instructions duration. The competencies which will be taught during this period are
enclosed herewith.
 Each student will be allotted to follow 4 patients in the ward
 Students undergo training in skill lab for 3hrsineach week (Every Saturday)
to attain proficiency in various skill competencies.
 Students will be trained to assess the clinical status of patient and
communicate any change in patient status to the unit in charge
 As per the curriculum students will be trained in various bedside procedures
to fulfil the desired no. Of competencies and record the same in their
logbooks.
Role &  Students will take proper history and do physical examination of the given
Responsibilities patients in the ward daily.
 Apart from this, students will be made familiar to the routine OPD services
during the admission day of respective unit and follow the admitted cases.
 Students will be actively involved in the follow up management of patients
allotted to them regarding investigations and treatment protocol.
 Students will analyze the investigation reports and review diagnosis and
treatment plan if needed and report to the Unit Incharge.

Clinical Postings:  During Phase II: 15 hours a week (3h/d for 5d/week)
 During phase III part I and Part II: 18h a week (3h/d for 6d/week).
 Clinical postings of General Medicine include clinical laboratory (in
Phase II) and infectiousdiseases (in phase III part I).
 Clinical postings of OBG includes maternity training, family welfare and
family planning.
 Clinical postings of Orthopaedics includes physical training and
rehabilitation.
 Clinical postings of Radio-diagnosis includes radio-therapy wherever
available.
Learner-Doctor  The learner becomes a part of the healthcare team.
Method  Although the learner shall not be given independent charge of the
patient, he/she shall be assigned patients.
 Learner shall remain with the admission unit until 6.00 pm (Except the
designated classhours).
 Learner shall be a part of the rounds at least on one other day other
than admission.
 Learner shall discuss the ethical and other humanitarian issues during
the rounds.
 Learner shall follow the patient’s progress until discharge.
 Learner shall participate in procedures for the assigned patients under
supervision
 Learner shall document observations in the logbook.
 One Mandatory round once a week to senior faculty by UG Students.
The focus of the  Phase I- Introduction to hospital environment, early clinical exposure,
doctor-learner understanding
method shall be as  Phase II - History taking, physical examination, assessment of change
follows: in clinical status,
 Phase II Part I - All of the above and choice of investigations, basic
procedures and continuity of care
 Phase III Part II - All of the above and decision making, management
and outcomes
Assessment  Logbook shall be assessed by a designated faculty for its
completeness, timely submission,quality of the report on the patient
assigned and the outpatients. Completed logbooks shall
form an eligibility criteria to appear in the final examination of the
subject.
Responsibility of  The faculty will teach them the proper history taking and physical
Concern Clinical examination during the case presentation by the students.
Departments  Students will learn communication skills by reflection with patients and their
family members by explaining prognosis, treatment, treatment outcomes
and patient education by the faculty.

GENERAL INSTRUCTIONS FOR PATINET GUIDE


DOs :-
 Be fully acquainted with these orders.
 Maintain absolute integrity at all times.
 Maintain absolute devotion to duty at all times.
 Constantly strive to improve professional knowledge and skills and bring about improvements in
health care delivery.
 To Do work promptly without delay.
 Provide health care of desired standards determined by the best practices in the profession.
 Be courteous towards patients, colleagues and public and respectful to superiors. When a conflict
has arisen or likely to arise with Patient – their Relatives, Beneficiaries and Visitors, DO NOT get
involved or aggravate the situation further. Report the matter to higher authorities for resolution of
the same
 In emergencies. No one is permitted to avail leave or leave the station until the competent authority
duly sanctions the leave applied for.
 Exercise economy in the use of lights, fans, fuel, stationary, drugs, dressings, transport and other
items. .
DON’Ts:-
 DO NOT discuss the diagnosis, laboratory reports or condition of the patients with unauthorised
person.
 DO NOT divulge matters of confidential nature to the patients or their relatives.
 DO NOT make loud conversation or other unnecessary noises within the hospital premises.
