CERTIFICATE OF NON-COMPLIANCE ON AMICABLE SETTLEMENT

There was an agreement set between the two disputing parties before the office of
the Punong Barangay on September 15 2023. On the said agreement, the respondent has
an obligation to the complainant amounting to Twenty Four Thousand Pesos (Php
24,000.00) plus damages and bills and on their agreement, he/she promised to pay Three
Thousand Pesos (Php3,000.00) every 15th and 30 th of the month and it will start on
September 30, 2023 and will last only 2 months to pay (November 2023) and the respondent
agreed to it. However, the respondent did not pay and did not appear.

Therefore, the complainant can now elevate this case to proper court, since the
respondent did not comply on their agreement, and in lieu of Certificate to file action
(CFA).

Issued this 31st day of May 2024 at Barangay Mabayuan, Olongapo City.
