City, after having been duly sworn to in accordance with law, hereby depose
and state that:

1. I am the owner of the property and business BREZY COXY HOMES


located and at Bokawkawn Road, Baguio City, 2600.

2. BONGA, who is residing at the aforementioned address, has a daily


water consumption of 80 grams.

3. This affidavit is executed for the purpose of providing an official record


of the daily water demand computation for the subject property and for
any legal purposes it may serve.

4. I am fully aware of the legal implications of providing false
