We would like to inform you that consent is hereby granted to our son/daughter

a requirement of the course Bachelor of Science in Information Technology, that will
cover six hundred (600) hours and will be conducted in

We are aware that the Immediate Job Supervisor within the company will take all

we will not hold Northwest Samar State University as well as the
