III) INSTRUCTIONS

This log book is intended to be a record of your basic academic and surgical training
with particular reference to your orthopaedic decision making and case management
including your operative experience.

1. You are expected to list, in chronological order, the training posts which you have
completed.

2. You are requested to record a surgical operations in which you have been personally
involved during your tenure of training posts. Each page of this record must be
countersigned by your unit head at the end of every month.

3. The operation record including the date of operation, the patient’s hospital indoor number,
the nature of the procedure, indication & whether it was performed under supervision or
as assistant.

4. You are required to record various academic activities in which you have been involved
/attended in each of your training posts. This record must include publications and
contribution to clinical and scientific meetings, attendance at meeting and training courses
and involvement in research projects.

5. The purpose of this log book is to have an audit of resident’s performance as well as ensuring
adequate surgical training. It is summative assessment process before obtaining the post
graduate degree/ diploma in orthopaedics.

6. Various sections of the log book should be filled from time to time. The residents should
obtain the signature of the concerned faculty every week.

7. The log book should be submitted to the head of the department at the end of every term

CLINICAL PROLOGUE
This post graduate log book encompasses the various competencies

achieved by the post graduate in the three year duration along with the

academic, clinical research, outreach activities and co – curricular activities.

This log book is divided into three years.

In the log book, section-A is on competency acquired by the junior

resident. The desired competencies are divided into three categories. They

are observed, performed under supervision and performed independently.

The aim of the whole exercise is to train a postgraduate from a novice to an

expert with constant and supervised training. The post graduate is expected

to fill the logbook regularly and under supervision of a faculty / guide.

The second (section-B) is on the academic activities that are conducted

regularly by the department as per the guidelines of NMC. The postgraduate

should enter all their academic activities in the log book, which they have

presented or participated which shall be initiated by the moderating faculty.

The third (section-C) contains the clinical activities which include the

bed side clinics and grand rounds.


The fourth (section-D) also has entry for the research articles

(publications) published in journals, conferences participated and presented.

The fifth (section-E) includes the institutional programs such as death

audit and scientific society meetings.

The sixth (section - F) includes the classes taken by postgraduates for

the undergraduate students. The entry should include the date, time and type

of academic program.

The seventh (section- G) includes the outreach and co – curricular

activities such as camps, red ribbon club activities etc.
