NOTE:

• This permit is automatically cancelled when there is an emergency or change of work other than
the original work plan. A new permit is required prior to the resumption of work.
• This is valid for 7 days
• If the provision of the permit is not followed the permit will be cancelled and the work supervisor
will subject for suspension/ termination from the project.
• The permit shall be closed out by all signatures and handed over to the HSE officer after the
completion of the task/ job.
