KNOWN TO ALL MEN BY THIS PRESENTS;

I, (NAME OF PRINCIPAL) of legal age, Filipino citizen and a resident of (ADDRESS) have
named, constituted and appointed, and by these presents do name and appoint to be my true and lawful
Attorney-in-Fact, (NAME OF ATTORNEY IN FACT) of legal age, single, Filipino citizen and
resident of (ADDRESS) for my name, place and instead and for my own use and benefits to do and
perform all or any of the following acts and things, namely;
1. To represent, receive/claim any documents or checks.
