KNOWN TO ALL MEN BY THIS PRESENTS;

I, (NAME OF PRINCIPAL) of legal age, Filipino citizen and a resident of (ADDRESS) have
named, constituted and appointed, and by these presents do name and appoint to be my true and lawful
Attorney-in-Fact, (NAME OF ATTORNEY IN FACT) of legal age, single, Filipino citizen and
resident of (ADDRESS) for my name, place and instead and for my own use and benefits to do and
perform all or any of the following acts and things, namely;
1. To represent, receive/claim any documents or checks.

2. To make, sign and execute, for and in my behalf, any documents which may be necessary
for the release of check;

3. To receive and in cash the check;

4. In general, to do all other acts things, or deeds required or necessary to successfully carry
out all or any of the above authorization.

GIVING AND GRANTING unto my said Attorney-in-Fact full power and authorities to do and
perform every act and the whatsoever requisite or necessary to be done in and about the premises as
fully to all intents and purposes as I might or could lawfully do if personally present and hereby ratifying
