I, CARMEN R. ANDRADE, of legal age, Filipino, widow, and resident
of Barangay Dayao, Roxas City, Capiz, Philippines, do hereby appoint,
name and constitute,
a.) MARK CHRISTIAN C. CANATA, of legal age, Filipino, married
and a resident of Barangay Tanza, Roxas City, Capiz, Philippines,
and/or;
b.) MARY JOY A. SAJISE, of legal age, Filipino, married and a
resident of Barangay Tina, Makato, Aklan, Philippines
to be my true and lawful Attorneys-in-Fact for me and in my name, place
and stead to perform the following acts and deeds and things, as follows:
To initiate, represent and file for and in my behalf in all transactions of
Perlite Security Agency with the Bureau of Internal Revenue (BIR)
relative to or in connection with all its business accounts, authority to print
invoices and receipts, and/ or processing of other BIR transactions and
documents, among others;
To initiate, represent and process business permit requirements and
all other transaction required to complete such task;
To sign, execute, receive and submit any documents in relation
thereto and;
To do or perform such other actions as may be necessary for the
accomplishment of above purposes.
HEREBY GIVING AND GRANTING unto my said Attorney-in-Fact full
power and authority to do and perform all and every act and things
whatever requisite or necessary to be done in and about the premises as
fully instants and purposes as I might or could lawfully do if personally
present, with full power of substitution and revocation, and hereby ratifying
and confirming all that any of the said Attorney-in-Fact shall lawfully do or
cause to be done by virtue hereof.
