I Rahul S/o Shiv Kumar resident of H no.410 Near Colony Ward No. 2, Vill. Barthal (35), Karnal, Pin:
132157 Haryana, India, do hereby solemnly affirm and declare as under: -

1) That I am citizen of India.


2) That I am going to Australia for Tourist Visa. That I am being support by my Self.
3) That all the documents I am providing for my Australia Tourist visa like the Business
certificate, Bank statement are genuine and correct.
4) That all the documents provided by me are genuine and have been so cured from the
related sources directly by me.
5) That if any document found false and fake, I surely personally responsible for that and is
liable to be punished according to law.
