AFFIDAVIT OF TWO DISINTERESTED PERSONS
We, EVELYN GONZALES and CORAZON CARPIO, both of legal age,
married, Filipinos, residents of 2nd-31st Sts., Nazareth, Cagayan
de Oro City, after having been duly sworn to law, hereby depose
and say:
1. That we personally have known spouses ROBERTO SATUR and
VILMA LEVISTE, since they have been our neighbors and close
family friends for a long time;
2. That we know for a fact that they were blessed with a
child in the name of ROVILYN LEVISTE SATUR, who was born on
September 27, 1983 at Cagayan de Oro City;
3. That however, her surname, was unintentionlly reported
and registered in the records of the City Civil Registry of
Cagayan de Oro as LEVISTE when it should have been SATUR as the
true surname of her father would reveal;
4. That since birth, in all of her public, official,
private as well as all of her scholastic records, she used SATUR
as her surname;
5. That therefore, ROVILYN LEVISTE SATUR, who was born on
September 27, 1983 and ROVILYN LEVISTE as appearing in her
Cerificate of Live Birth, refers to one and the same person;
6. That we are executing this affidavit to attest to the
veracity of the foregoing facts and statements for whatever
legitimate purpose this may serve.
