AFFIDAVIT OF LOSS
I, CARL R. TORION, of legal age, Filipino, single, and a resident of
Purok Maliwanag, Barangay Sumo-sumo, Municipality of Tago, Surigao del
Sur, state under oath, THAT:
1. That my uncle Lenjhun T. Torion iss the owner of the lost motorcycle
plate number card 527ZJB, issued at Land Transportation Office in Tandag
City, Surigao del Sur;
2. That on October 29, 2024, I was driving the motorcycle along
Barangay Sumo-Sumo Municipality of Tago Surigao del Sur going to Nemsu
to attend my classes and I parked the motorcycle near our CITE Department;
3.That after attending my last class at 8:30 pm, I went to the
motorcycle where I noticed that its plate number was missing;
4. Thereafter I immediately asked the security guard of Nemsu if he
had seen the motorcycle plate number but he had seen none;
5. I tried to access the CCTV cameras surrounding Nemsu University if
someone took the motorcycle plate number or someone had seen it but it
was futile;
6. That I diligently searched for the motorcycle plate number in my
house but no avail as well and without any negligence on my part;
7. I am executing this Affidavit to apprise on the fact of said loss, attest
to the truth of the foregoing facts, and for whatever legal purpose this may
serve best.
