National Honor Society Intent to Apply Form

We have a digital application process.
This is the only piece of paper that gets submitted, the rest will be done in Microsoft
Forms.
The link to apply can be found on the Central Bucks East NHS website under the heading
NHS Application Materials.
Carefully read and follow all directions on the digital application.
Read all information and FAQ’s on the NHS website!
The student desiring membership must complete this form and turn it in to the NHS
advisors by the due date.
This paper and ALL digital forms must be submitted by Friday, September 6, 2024.
For each school activity, leadership position and community service activity, the
supervising adult must submit a complete and accurate National Honor Society Activity
Evaluation Form. The form must include the name and contact information of the
supervising adult. This must be completed by Thursday, September 20, 2024.
Family members cannot complete evaluation forms for applicants.
List activities that were completed after August of your freshman year. Activities that
were completed after August of your freshman year can be listed for in-school,
leadership or community service on your application.
A Faculty Selection Committee determines membership based on established criteria
(the four pillars of NHS):
1. In School Activities (as an ongoing commitment)
2. Service to Community (a minimum of 5 hours is required to apply) These hours will
not count towards required membership hours once inducted.
3. Leadership (an elected or appointed role, not babysitting, lifeguarding, etc..)
4. Good Character (within the past 12 months, no record of cheating, plagiarism,
intentional dishonesty, violating school rules or civil offenses in the community)
