OMH 551 Checklist

(For E-Z PAR Projects) Obtain a letter of support from the local
[ ] §551.5 (a) governmental unit or designee in the county(ies) to be served

For projects including acquisition, construction, substantial renovation, or


[ ] §551.10 (a) alteration, submit compliance info within 4 months of application approval

If the facility is leased, identify the owners of the premises, and if a


corporation, it's incorporators and directors. Provide lease with required
[ ] §551.10 (b) language
For New Construction & Substantial Alterations: (alterations exceeding 50%
[ ] §551.11 (b)(1) of the buildig's value):

The facility shall be designed and constructed to be readily accessible to and
useable by persons with physical disabilities.
The design of the faclity shall meet the most current requirements fo the ADA (28
CFR part 35 and 36)

No less than 5 percent of the facility's occupancy, or at least one bedroom
(whichever is greater) shall be accessible

§584.19 (a)(1) Facilities shall be in compliance with 2010 FGI Guidelines


§584.19 (a)(2) Facilities shall meet the requirements of ADA
§584.19 (a)(3) Up to 10% of minimum SF may be waived in bedrooms

All buildings containing sleeping quarters for children or youth shall be protected by
a fire detection system or a sprinkler system installed throughout. All buildings used
by children or youth, but not containing sleeping quarters for them shall be
protected throughout by a sprinkler system, fire detection system, or manually
operated fire alarm system. All areas of high fire hazard in all buildings used by
children or youth, whether or not they contain sleeping quarters for them, shall be
protected by a sprinkler system and be separated from other areas by substantial,
§584.19 (b)(1) fire-resistant construction.
§584.19 (b)(2) Fire protection systems shall be incompliance with NFPA.
Devices for heating, cooking, or lighting which use kerosene, gasoline, wood, or
§584.19 (c)(1)(i) alcohol are prohibited
§584.19 (c)(1)(ii) Portable electric hot plates are prohibited

Barbeque grills are prohibited. The use of barbeque grills is permissible when used
outside of buildings but not within 30 feet of any structure including overhangs,
§584.19 (c)(1)(iii) canopies or awnings.
Medication shall be stored in a sturdy metal or sturdy wooden cabinet without
§584.19 (2)(e) glazing which shall be locked except when medication is needed.

Schedule I, II, III and IV controlled substances shall be kept in stationary, double-
locked cabinets. Both inner and outer cabinets shall have key-locked doors with
§584.19 (2)(e)(i) separate keys
