AFFIDAVIT OF LOSS
I, JENNIFER CALIMAG TABAO, of legal age, Filipino with residence
and postal address at Camasi,Peñablanca, Cagayan, after having been sworn to in
accordance with law, states:
1. That I am a possessor of Student Permit issued by Land Transportation
Office;
2. That I placed the said Student Permit in my black wallet while driving
my motorcycle and when I tried to retrieve the said wallet which contained my
Student Permit unfortunately it is nowhere to be found;
3. That from the foregoing circumstances and despite diligent efforts to
locate the said student permit, in my knowledge it is beyond recovery and
considered lost for all intents and purposes;
4. That this affidavit is executed to affirm the truth of the foregoing
statements for the purpose informing the Land Transportation Office and for any
legal purposes it may serve.
