SUBJECT: AUTHORIZATION LETTER

To Whom It May Concern:

This is to authorize my mother, CECILIA GALAMAY BUGTONG, to process and claim


my educational assistance. Due to reason for personal commitments, I am unable to be
present to perform this task personally. Therefore, I have granted my mother, CECILIA
GALAMAY BUGTONG, full authorization to act on my behalf. This includes the
signing of documents, receipt of the item or document, and any other actions necessary
for the claim.

Attached to this letter, you will find a photocopy of my identification for verification
purposes. Additionally, my mother will present her identification ID upon request.
