NCP Sleep Pattern

The patient reports sometimes falling asleep but then being suddenly awakened or touched, making sleep difficult. The nursing diagnosis is a disturbed sleep pattern related to a lack of privacy during sleep as shown by reports of being awakened and dissatisfaction with sleep. The hospital environment can be noisy and special care needs to reduce noise to help or hinder sleep, as any changes in the environment like noise can inhibit sleep. The goal is for the patient to report improved sleep after 2 days of interventions like adjusting lighting, maintaining a quiet environment, minimizing disturbing factors, and lessening visits to provide more privacy and relaxation.

ANALYSIS Environment can promote or hinder sleep. Any change, for example noise in the environment can inhibit sleep. The absence of usual stimuli or the presence of unfamiliar stimuli can prevent people from sleeping. Hospital environment can be quite noisy, and special care needs to reduce noise. (Kozier & Erbs Fundamentals of Nursing 8th edition, volume 2 pg. 1170)
