To whom it may concern,
I am writing to you on behalf of ANT TRUCKING DISPATCHING LLCto discuss the actions that we
are taking to improve our safety compliance program and CSA scores. ANT TRUCKING
DISPATCHING LLChas recently hired the Simplex Group to work alongside our staff as our outside
safety department.
The Simplex Group will be working with us to ensure we are compliant with all FMCSA requirements
and is in the process of creating policies and procedures for our company to implement with our drivers.
Below is a description of the services that will be provided toANT TRUCKING DISPATCHING LLC by
the Simplex Group, Inc.
Dedicated Compliance Representative
Simplex has assigned a dedicated Client Service Safety Representative to the account. This safety
professional is responsible for working with ANT TRUCKING DISPATCHING LLC weekly to ensure
on-going compliance.
During each conference, the following items are discussed and/or reviewed:
• Driver Qualification
• Driver Documentation expirations
• Vehicle Documentation expirations
• Vehicle Preventive Maintenance
• Random Testing program
• Accidents
• CSA Scores
• Hour of Service Reports
• Negative drivers
• PSP Monitoring
• Pending Polices and Procedures
Every service call is followed up with a service report detailing the current status of our Fleet Safety
Compliance Program.
DOT Safety Operation Manual
A complete “Safety Operation Manual” is being implemented and customized to our operational needs.
This manual covers the creation and management of an Accident Review Committee. In addition, the
manual provides the implementation of a disciplinary procedure for drivers involved in safety violations.
All current drivers have provided a copy and have signed an acknowledgement of understanding. This
process is included for all new hires, during the new hire orientation. Our hiring criteria are as follows:
• Driver must be approved by insurance
• PSP is ran on driver to verify roadside inspection history
• Driver must have 2 years of verified CDL experience
• Driver must be 25 years and no older than 70 years old
• Drivers over 60 must provide a medical long form
• Drivers with 2 or more at fault accidents are not hired
• No more than 3 moving violations in 2 years
• No major violations such as reckless driving, DUI
Substance Abuse Program
The Substance Abuse Program is managed by Simplex Group, meeting all requirements as per DOT
regulations. Simplex management of the Substance Abuse Program includes but is not limited to, Pre-
employment testing, random selection and post accident testing.
Driver Qualification Files
A complete Driver Qualification Program has been implemented, as per DOT regulations and will be
monitored on an on-going basis for compliance by Simplex Client Safety Service Representative. All new
drivers will be processed at Simplex to ensure full compliance.
Simplex will process all past-employer requests, as per DOT regulations, within 30 days of the driver
being hired. Any negative feedback will be forwarded to our safety representative.
Preventive Vehicle Maintenance
With the assistance of Simplex, a complete preventive vehicle maintenance program has been
implemented as per DOT regulations and will be monitored on an on-going basis for compliance by
Simplex Client Safety Service Representatives.
Driver Time Logs
Driver time logs are being audited to ensure compliance. Simplex is in the process of preparing monthly
management reports with the purpose of identifying potential and existing violations. Simplex will
electronically audit all logs and reports findings to ANT TRUCKING DISPATCHING LLCmanagement
team. Through Simplex, we also provide training to dispatch and drivers within the specifications of these
requirements.

LLC for driver training purposes:

Safety Management Commitment and Involvement
As a company policy, each driver will receive a copy of the Safety Management Commitment and
Involvement policy. This statement dictates how important safety is and how devoted management is
towards maintaining safe practices and procedures within our operation.
