Sub: Request for the disconnection of BSNL broadband

Dear Sir/Madam,

I have been using the BSNL broadband connection for the past four years with the customer I(01765296054),
and I have been really grateful for the services provided by BSNL. Now since I have to shift to my hometown, I
would like to cancel my connection.

Along with the letter, I have enclosed the details of my address proof and the last three months’ payment
receipts. Kindly consider my request and cancel my connection at the earliest so that it can be convenient for
me. I shall be highly obliged for this kind gesture of yours.
