SOAP Note Assignment Guidelines
Documentation is an essential part of healthcare provides, regardless of
your career track (e.g., OT, PT, SLP, RC). This assignment has two parts;
however, only one paper is needed.
The first part is to research what is a SOAP note. Write at least a
page describing a SOAP note and why it is essential. Use APA guidelines in
your paper, including references/citations.
The second part is applying SOAP note. There are various case study
examples you can search online. Find an appropriate case study (in your
field) or use a hypothetical case example of a patient/client. Never use
real patient/client information. Document the case based on
SOAP note, as a practitioner.
The paper should have a cover page, a reference page, and it should be
at least 2 – 3 pages (not including the cover and reference
pages), Times New Roman, 12pt, double-spaced,
1) What is a SOAP note?
2) Case study: present the case example. Do not use names.
3) SOAP note details that you can demonstrate your understanding of
the case followed by SOAP note technique.
Extra tips:
1. Maintain a professional voice.
2. Avoid overly wordy phrasing.
3. Avoid biased overly positive or negative phrasing.
4. Be specific and concise.
5. Avoid overly subjective statement without evidence.
6. Avoid pronoun confusion.
7. Be accurate but nonjudgmental.
