Exit Clearance Process Guide
1. Surrender all Company issued items:
ID Badge - surrender to the Security personnel at the front desk ground floor
If lost, provide an Affidavit of Loss indicating the specific item/s that was/were lost
Headset
if account managed - return to the account POC (previous Supervisor/Trainer)
for non-account managed - return to IT team
Locker
proceed to Facilities Office Phase 2, 5th floor
WAHA Accountabilities (if applicable)
should be surrendered to Operations/IT/FAMU
coordinate with the account leaders to lodge TPWAHMS ticket

2. Fill out the Exit Clearance Monitoring Sheet at the Ground Floor Front Desk
**Note: COE and clearance letter will be sent via email within 24 hours.
To follow up with us on or before the thirtieth (30th) day from the date employee has processed the exit
clearance by calling TPLink hotline #8353 or sending an email to [email protected].

If an authorized representative will process this, the latter should provide the following:
a. Authorization Letter
b. Photocopy of 2 valid IDs of the representative
c. Photocopy of 2 valid IDs of the employee
