AFFIDAVIT OF NO EARNINGS

I, _____________________, Filipino, of legal age, married, and a


resident of Rizal Avenue, Molave, Zamboanga del Sur, Philippines,
after having been duly sworn to in accordance with law, hereby
depose and say, THAT:

1. I am a member of the Social Security System (SSS);

2. I am self-employed and I have no more job and neither did I


have any gainful means of livelihood since June 30, 2017;

3. I am dependent for support from my children;

4. This affidavit is executed in order to state for the records the


facts of herein stated and for any legal purpose it may serve.
