JOINT AFFIDAVIT OF CHANGE COLOR

I/We, CHARITO R. BUNDOC, JR. and MARLON G. RAPIN, both legal


age, Filipino Citizens, and residents at Tamorong, Sta. Catalina, Ilocos Sur,
and Bahet, San Ildefonso, Ilocos Sur, respectively, after being duly sworn to
in accordance with law, do hereby depose and say;

That I, CHARITO R. BUNDOC, JR., am the registered owner of the


certain motor vehicle which is more particularly described as follows, to wit:

COLOR………………………….. BLACK

That said motor vehicle’s original color is BLACK and I decided to


change it to MULTIPLE COLORS;

That herein affiant MARLON G. RAPIN is an auto-mechanic/painter by


profession, that my service was hired by CHARITO R. BUNDOC, [Link] do
the necessary change of color of his motor vehicle from BLACK to
MULTIPLE COLORS.

That we are executing this joint affidavit to attest to the truth of all the
foregoing facts and for the purpose of informing the authorities concerned of
such changes.

IN WITNESS WHEREOF, WE have hereunto set our hands this 4 th day


of January, 2021 at Vigan City, Ilocos Sur.
