CONTRACT VARIATION
Pursuant to Terms and Conditions under Clause 1 stipulated in the Service Contract for Registration
Center Supervisor effective 01 August 2022 to 30 September 2022 including Saturdays, Sundays,
and Holidays as needed executed on the 29th day of July 2022 under Notarial Docket No. 211,
Page No.44, Book No. XI, Series of 2022, the contract is hereby amended as follows:
1. Clause 1, page 1 of the Service Contract stating “The PSA shall hire the Registration Center
Supervisor effective 01 August 2022 to 30 September 2022 including Saturdays,
Sundays, and Holidays as needed” is hereby amended to:
“The PSA shall hire the Registration Center Supervisor effective 04 July 2022 – 01 October
2022 including Saturdays, Sundays, and Holidays as needed,” based on Office
Memorandum No. 2022-135 Philippine Identification System Step 2 Registration from July to
September 2022 with reference no. 22ROS04-06-084 dated 20 June 2022.
2. All terms and provisions of the Agreement not amended hereby, either expressly or by
necessary implication, shall remain in full force and effect.
