MEMORANDUM OF AGREEMENT
(On-the-Job Training Program)
WITNESSETH, that
WHEREAS, ASC is offering a course of Bachelor of Science in Agriculture (Animal
and Crop science), and has instituted an ON-THE-JOB TRAINING PROGRAM where
qualified students will undergo training as part of the curriculum in order to be
adequately familiar with actual work and to augment their formal training;

WHEREAS, ASC proposed that its students be allowed to undergo On-the-job Training
Program with the HOST TRAINING AGENCY which proposal was accepted by the
latter as part of its commitment to upgrade the skill and working knowledge of students,
subject to the terms and conditions provided for in this agreement;
NOW THEREFORE, for and in consideration of the foregoing premises, ASC and the
HOST TRAINING AGENCY agree and stipulate on the following:
a. Design a training plan specifying therein the goals and objectives, knowledge,
skills, attitudes, and competencies which the student-trainee must acquire in each
learning area, assignments, and schedule of activities, among others;
b. Monitor and evaluate the performance of the student-trainee based on the
prescribed training plan;
c. Monitor the student-trainee and attend to his/her needs;
d. Conduct post-training review and evaluation of the program and the performance
of student-trainees in accordance with the agreed evaluation system for student
performance;
b. Conduct pre-departure orientation/seminar for the students who shall undergo the
OJT program in the Host Training Agency.
(2) Conduct initial and regular visits/inspections of their premises to ensure the
safety of students;
e. Furnish the Host Training Agency with the evaluation system to be followed in the
evaluation of student performance;
f. Monitor compatibility of job circumstances with the course requirements for the
student’s attainment of advanced standing in the program through regular
interaction with the students;
d. At the end of the training period, the HTE shall transmit to the ASC the following
documents:
(1) Certificate of completion to the student-trainee who has successfully finished
his/her training in the OJT program;
(2) Duly accomplished evaluation sheet; and
(3) Other pertinent reports, information, and/or documents that may be included for
purposes of describing the student trainee’s performance;
e. Allow periodic visits by the ASC'S designated OJT coordinator;
written notice to the other party or discontinue the OJT of any one of the student-
interns subject to the observance of due process.
3. In case any one or more of the provisions contained herein shall, for any reason be
held to be invalid, illegal or unenforceable in any respect, such invalidity,
illegality or unenforceability shall not affect any of the provisions of this
agreement. In such event, the parties shall consult each other as to the manner by
which their original intention can be fulfilled as closely as possible and they will
amend this agreement accordingly.
4. This Agreement may be amended, modified, novated or altered only in writing
duly signed by all the parties.
ACNOWLEDGEMENT
This instrument, consisting of four (4) pages, including the page in which this

WHEREAS, ASC proposed that its students be allowed to undergo On-the-job Training
Program with the HOST TRAINING AGENCY which proposal was accepted by the

subject to the terms and conditions provided for in this agreement;

ge 3. THE HOST TRAINING AGENCY SHALL:

a. Designate from among its employees a supervisor who will be assigned the
following responsibilities:
(1) Serve as mentor to the student–trainees;
(2) Give the student trainee course-related work assignments and expose them to the
learning experiences on actual operations and management;
(3) Expose the student-trainee to a real situation/problem thus enabling them to
analyze and apply scientific as well as empirical approaches in solving technical
problems;
(4) Arrange a conference with the Program Coordinator should a student-trainee
problem arises;
b. Provide Supervised Applied Learning Experience (SALE) for the student-trainee in
accordance with the agreed training manual/plan and schedule of activities;
c. Accomplish the necessary forms required by the ASC, in connection with the

training of student-trainee;
d. At the end of the training period, the HTE shall transmit to the ASC the following
documents:

h. Adhere to all Philippine laws, rules, and regulations pertaining to the student
internship programs, as well as health and safety laws applicable to entities
similarly situated;
i. Observe health and safety protocols for students during their stay in the HTE and
ensure the safety of the student-trainee while he/she undergoes the training program
1. The ASC does hereby hold free and harmless from any liability and will not file
any claim against the HOST TRAINING AGENCY, its officials, and employees
for any and all harm, injury, or loss that may come upon the person or property of

the STUDENT-TRAINEE on his/her way to, and from, and while inside the site
of the HOST TRAINING AGENCY during the OJT/internship period, arising
from the fault or negligence of the students in the performance of their duties and
functions while undergoing the OJT program, subject to existing laws, rules and
regulations. However, this shall not include any injury, loss or damage suffered by
the STUDENT-TRAINEE which is attributable to the fault or negligence of the
HOST TRAINING AGENCY, its officers, employees or representatives.
2. No form of relationship not expressly stipulated herein shall be inferred here from
and none of the parties is authorized to represent or bind the other party.
3. That both parties shall exert all efforts necessary to achieve the objectives of this
joint undertaking. For this purpose, the parties shall execute and deliver such
instruments and documents which may be reasonably requested by the other party
in order to carry out the intent and accomplish the purpose of this agreement and
transactions covered hereby.
