AFFIDAVIT OF LOSS

I, GLYN M. LOYOLA, of legal age, single and a resident of


Pascuala Road, Santo Rosario, Capas, Tarlac, after having been duly
sworn to in accordance with law, hereby depose and state that:

1. I am the same GLYN M. LOYOLA, born on April 19, 1986 in Leyte


by Sps. Narciso M. Loyola and Avelina M. Maria;

2. When I secured PSA authenticated copy of my Certificate of Live


Birth, I found out that the entry for my Sex/Gender was
erroneously indicated as FEMALE when in truth and in fact that I
am a MALE;

3. I am executing this affidavit to attest to the truth of the


foregoing and for whatever legal purposes the same may serve.
