General Instruction to the candidates for Physical verification

The candidates should bring their original testimonials as listed below for verification purpose at the venue.
1. On-line application print-out duly signed by the candidates with date (after submission of application)
2. Original Photo Identity Proof (issued by competent authority)
3. Original copy of
a) Birth Certificate/Admit card or equivalent of Class-X as age proof
b) All the Mark sheets (for all years/semester) and course completion certificate of all essential and desirable
qualifrcations .
c) Caste Certihcate issued by competent authority
d) CouncilRegistrationCertificate
e) PPO in case of retired Govt employee only
f) Experience will be calculated after obtaining the required essential academic qualification for the mentioned
position. Offer or appointment/engagement letter will not be treated as experience. Experience of a number
of years is prescribed as an item of qualifrcation this would mean only experience gained in full time
employment in the relevant field within lst Jan 2024. Experience certificate must consist of Name of the
post hold, Employer's nanne, Employee's name, date of joining & date of leaving/current date in case of still
continuing otherwise his/herexperience will be treated as invalid. Ph.D/M.Phil will no
experience period. Existing NHM employees should submit experience certificate with above mentioned
criteria and mentioning their HRMIS ID.

2) If the online application details submitted by the applicant differ with the original testimonials, that application
shall be liable to be cancelled.
