D. Presenting examples/instances of the We are oftentimes asked to fill out forms whenever we use documents at the school, bank, or office. This is an essential activity
new lesson/Motivation that we cannot avoid doing. For these reasons, we need to master how to fill out forms. Can you do the job? Did you know that
to fill up CANNOT be used to mean the act of completing a form? Can a form hold liquid? Since it cannot, it is not appropriate to
use fill up. The appropriate expressions are fill in and fill out. So, please don’t ask anyone to fill up a form because they will not
be able to do it. Ask them to fill in or fill out a form instead.
E. Discussing new concepts and practicing Forms are everywhere, at school, at work, banks, hospitals, offices, restaurants, when paying bills and even when you do online
new skills No. 1 shopping. Forms are part of our daily lives. As you grow older you will be encountering more kinds of it. It is good for you to know
as early as now how to fill out form accurately and correctly. To fill out means to complete a form by writing the required
information on it. What are these information? It can be your name, birthday, address, Learner’s Reference Number (LRN),
contact number, and many more.
F. Discussing new concepts and Form is a structured document with spaces or blank field in which you can write on, check or select the required information. It is
practicing new skills No. 2 used to gather necessary information for a particular purpose. Forms usually ask you to give details about yourself. There are
particular forms that you have to fill out as a requirement for admission to a school, enrolment form at the beginning of the school
year or registration form in an organization or school club. When transacting in a bank you will fill out cash transaction slip,
deposit or withdrawal slip. When you finish your studies, you will be applying for a job and you need to fill out an application form
or a Bio-data. There are also electronic forms like google forms which are widely used nowadays as these can be filled out if you
are online. Forms are filled out in either single, duplicate or triplicate copies depending on the requirement set by the institution.
Accuracy is very important in filling out these forms. Did you know that to fill up CANNOT be used to mean the act of completing
a form? Can a form hold liquid? Since it cannot, it is not appropriate to use fill up. The appropriate expressions are fill in and fill
out. So, please don’t ask anyone to fill up a form because they will not be able to do it. Ask them to fill in or fill out a form instead.

In filling out forms we need to:


• Read and analyze each item and instructions in the form carefully.
• Follow the given directions precisely.
• Provide true and correct information.
• Write legibly and neatly. Avoid making erasures and any stray marks.
• Do not use red or other ink colors aside from the standard blue or black.
• Do not skip any sections. Remember to fill out all applicable sections.
• Handle the form with utmost care.
• Review the entries or date you have entered before submitting the form.
• Submit the accomplished form to the authorized person who will collect it. Common instructions in filling out forms:
• Some information should be written in CAPITAL LETTERS.
• Write N/A if the items are not applicable.
• Some forms require a pattern on how you will write the needed information.

Example of patterns when writing your name

A registration form is a list required information that a user will complete to be submitted to a company, an organization or an
individual.
Example of Club Registration Form.

A deposit slip is a small paper form that bank customer includes with a bank deposit to show how much money he or she is
putting in a bank account. It contains the deposit date, name and bank account number of the depositor and the amount to be
deposited in the form of checks or cash and the signature of the depositor. Specifically, a person who deposits or places money
in a bank account is called a depositor. How to fill out a deposit slip 1. First, write your account name and bank account number.
2. Then, supply additional details such as transaction date and any branch information required.
3. List the cash amount breakdown of your deposit and the total amount thereafter.
4. Affix your signature over printed name found on the lower part of the form.
A withdrawal slip is a small paper form that has to be filled out before making a withdrawal money from a bank.
How to fill out a withdrawal slip
1. First, write your account name, account number and the date.
2. Then, write the desired amount of your withdrawal in words and in figures.
3. Finally, affix your signature on the signature line but do this only until a bank teller can witness you signing it.
