:The checklist shall be completed by WPR prior to any night work activity and shall be verified by the Permit issuer and the Area supervisor. The checklist provides
minimum requirements to comply for any extended/night work activity.
If any of the basic requirements will not be complied with, the extended work or night shift permit shall not be approved or validated
