I am referring 48 years old Mr. Elvin, whose features are suggestive of a myocardial infarction and an
asthma exacerbation, for emergency medical care.
Today, Mr. Elvin presented with a sudden onset crushing chest pain along with worsening asthma. His
examination revealed a gallop rhythm, wheeze, bilateral crackles and an oxygen saturation of 85% at room
air. His ECG showed mild ST elevation in anterior leads and a ST of 120. Therefore, intravenous morphine
5mg, frusemide 40mg, GTN patch, ipratropium 500ug nebulization was given to him and oxygen inhalation Commented [OET2]: was
started at 15 liters while transfer to emergency department was arranged.
Mr. Elvin first visited the practice on 05/05/11 with the concern of occasional mild central chest pain on
exertion. A review of his investigations, one week later, indicated mild hypercholesterolemia and slight
ischemic changes on exercise testing. Hence, he was commenced on Lipitor, Imdur, aspirin and Anginine.
Mr. Elvin was also having recurrent mild asthma exacerbations and forgetfulness regarding his asthma
medication owing to his excessive alcohol consumption. Thus, he was counselled regarding medication
compliance and alcohol dependence pharmacotherapy was offered to him.
Please note that Mr. Elvin smokes 1 pack of cigarettes daily and drinks 10 standard drinks five days per
week. He has been taking salbutamol and Seretide for his asthma.
