AN EXECUTIVE ORDER RE-ORGANIZING AND CREATING THE CONTACT TRACING
TEAM (BCTT) OF BARANGAY GAYA-GAYA, CITY OF SAN JOSE DEL MONTE, BULACAN
WHEREAS, Section 16, or the General Welfare Clause, of the Local Government Code of 1991, the local
government units are charged with the promotion of health and safety within their respective territorial
jurisdiction;
WHEREAS, Republic Act. No. 11469, or the “Bayanihan to heal as One Act”, has placed the country in a
State of National Emergency, and has declared the urgent need to; (a) mitigate, if not contain the
transmission of COVID-19; (b) undertake measures that will prevent the overburdening of the healthcare
system; (c) immediately and amply provide healthcare, including medical tests and treatments, to COVID-19
patients, persons under investigations (PUIs), or persons under monitoring (PUMs); (d) ensure that there is
sufficient, adequate and readily available funding to undertake the foregoing;
WHEREAS, the City Government of San Jose del Monte, Bulacan established and organized the City
COVID 19 Call Center (4C’s Project) under City Disaster Risk Reduction and Management and assigned all
Barangay to create its own Barangay Covid-19 Call Center who will assigned in monitoring and manage all
COVID-19 related calls;
WHEREAS, pursuant to City Resolution No. 2021-19-020 provides that all Barangays shall create its own
Barangay Contact Tracing Team;
NOW, THEREFORE, I ENRY L. SANTOS, Punong Barangay, by virtue of the powers vested in me by
law, do hereby re-organize and create a Barangay Contact Tracing Team (BCTT);
Section 1. Composition. “Barangay Contact Tracing Team (BCTT)” to be composed of the following:

Section 2. Duties and Functions of the Barangay Contact Tracing Team. The BCCT shall have the
following duties and functions:
1. Complete contact tracing to known Covid-19 positve up to 2nd and 3rd level generation;
2. Ensure that patients with Covid-19 cases are strictly observing appropriate home
quarantine measures or are properly situated in a government-managed isolation facility;
3. Shall monitor the status probable, suspected and confirmed cases including family
members with known contact to a confirmed case to mitigate the spread of the virus;
4. Shall report daily through the BHERT and the rest of the team (a mobile/non-mobile
based approach) supported by other volunteers and contact tracing personnel of close
contacts and newly arrived personalities such as (Returning OFWs and others who have
been exposed outside the City of SJDM);
5. Shall submit corresponding recommendation to the City Health Center/Unit those who
found close contact with confirmed patients, and others who are in need of medical
intervention or other equivalents;
6. Conduct a House-to-House COVID-19 monitoring and information dissemination;
7. Report daily to the Punong Barangay the conditions of the monitored individuals
including all measures and action taken.
Section 3. Effectivity. This Order shall take effect immediately.
