This document represents the iQ-4VIEW Administration Guide for software version 2.2 provided
by IMAGE Information Systems Europe GmbH.
This Administration Guide provides all necessary information regarding the installation and
licensing of the application as well as how the software is properly maintained to fulfill its
intended use.
It contains instructions concerning the configuration and set-up of the application as well as hints
regarding trouble-shooting problems. The Administration Guide covers the following topics,
among others:

iQ-4VIEW is a web application with direct access to a PACS (iQ-WEB) database, intended to be
used for digital reading, viewing and reviewing of diagnostic medical images.
It provides a number of image processing tools, but does not intent to replace a fully-equipped
diagnostic reading station. Due to its functional limitations the application does not target
radiologists whose full-time work it is to read medical imaging data. It is more common to be
Due to the nature of a web application the executing devices range from a desktop computer to
mobile devices such as tablets or even smartphones. Depending on the used hardware device
the application runs on, iQ-4VIEW can be used for image viewing, reviewing and reading
purposes. Because of the small screen size of smartphones and tablet computers (<13”) and the
resulting inaccurate handling, usage with diagnostic intent is not possible. Therefore the use on
such devices is restricted to viewing purposes only. The use on other devices where the
compliance with all regulatory requirements can be ensured may be for diagnostic purposes
(reading, reviewing).
The main target user base are physicians and veterinarians with radiological knowledge that
should receive a product training via user documentation and workshops of the manufacturer or
authorized sales partners.
iQ-4VIEW is intended for both routine and emergency diagnosis procedures, although its use for
emergency cases is limited to locations where a stable and reliable network and/or internet
connection exists to ensure the availability of the relevant diagnostic data.
NOTICE:
When the iQ-4VIEW software is used not for human medicine but for veterinary medicine,
the product does not fall under the European Council Directive 93/42/EEC and, therefore, does
not qualify as a medical device in the EEA. The regulations of the 93/42/EEC do not apply.
The purpose of this section is to define and explain the warnings and precautions used in this
document.

Although the iQ-4VIEW software is subjected to extensive validation and verification
procedures by the manufacturer, it is nevertheless possible that unforeseen display errors,
deviations in measurements or failing processing may arise during use of the software. Users
should at all times be aware and warned of such eventualities. In case faulty software behavior
is observed that may put a patient or the user at risk, the user is asked to immediately contact
the manufacturer or local reseller. The manufacturer’s contact data is stated in the user
documentation and inside the application itself.

The iQ-4VIEW software is certified as a medical device according to the European Council
Directive 93/42/EEC. Before using this application, make sure that you have thoroughly read
and fully understood the content of the Instructions for Use and Administration Guide,
including all of the warnings and notices.

This iQ-4VIEW software version can be integrated into more than just one iQ-WEB version.
Therefore, names and descriptions of options and functions relating to the web interface of iQ-
WEB may differ from the information given in these instructions for use depending on the iQ-
WEB version you have installed on your system. In case of questions contact your local
reseller.

This medical software does not replace the competence and judgment of qualified medical or
radiological personnel in any way. It should only be used by qualified and trained persons,
because any actions of the user may directly affect the accuracy of the functions and the
results acquired with the help of the software.

All modifications to the medical software have to be made by service personnel. The risk of
malfunction is relatively high when modifying software. Abnormal termination of the software
as well as temporary data losses are possible when improperly administered.
Modifying application folders and or moving files to different locations, deleting or renaming
them without considering other parts may cause problems in the functioning of iQ-4VIEW.
Keep the file and folder structure intact and only follow the user documentations’ instruction
for configuring the application.

Danger of unauthorized access to application.
An unauthorized access to the system may lead to non-functioning. To minimize this risk, the
system/device should be protected by a password so that only authorized persons will have
electronic access to the system. Authorized persons access the system using a password-
protected Windows account.

WARNING:
Danger of application failures or unavailability.
The capabilities and the performance of the software can be affected by limitations caused by
the hardware. Therefore an appropriate setup and maintenance of the hardware is necessary
(e.g. professional connection to power and network, sufficient ventilation, regular cleaning of
the fan, etc.). The computer/device should be shut down regularly for memory cleaning
purposes.

WARNING:
Danger of data loss/inconsistencies or application failures.
Computer viruses hold a considerable risk. A virus infection may potentially lead to data losses
and to the unavailability of the iQ-4VIEW viewer. To avoid the risk of a virus infection of the
computer/device, on which iQ-4VIEW is used, all systems/devices should be furnished with
anti-virus software that needs to be updated regularly.

WARNING:
Danger of misdiagnosis due to manipulated data.
The user should make sure that the whole chain of data flow from the acquisition device via
DICOM routers to the iQ-WEB archive to the iQ-4VIEW client does not affect the diagnostic
accuracy of received data, e.g. regarding pixel aspect ratios, compression artifacts, slice
thickness, etc.

WARNING:
Danger of incorrect patient demographics or study meta data.
The modality operator is responsible for the quality of the image data as well as the
correctness of the patient and study information. Insufficient quality can lead to misdiagnosis
while incorrect patient information can lead to wrong diagnosis for a respective patient (patient
mix).

WARNING:
Danger of misdiagnosis due to non-DICOM 3.0 compliant image data.
Use of DICOM dialects might lead to incompatibility and delay or even wrong diagnosis as the
system may not be able to read out the necessary image information to create the images and
patient/study information sent to the iQ-4VIEW clients.
System administrators should run a system integration test with each newly connected
acquisition device before clinical use.

WARNING:
Danger of misdiagnosis due to the use of lossy image compression.
Excessive compression levels may cause compression artifacts that might reduce the image
quality to non-diagnostic level. These images may no longer have diagnostic quality!
If the system detects the use of any lossy compressed images, the application marks these
images accordingly with a “Lossy compression” label. The user’s organization should make
sure to use lossy compression only according to local regulatory requirements.

WARNING:
For users under the jurisdiction of the U.S. Food and Drug Administration (FDA)
Danger of misdiagnosis due to lossy image compression in mammography.
Currently the FDA does not permit images regenerated from lossy compressed data to be used
in the same manner as the original mammogram. As a consequence, lossy compressed
mammography images must neither be reviewed for primary diagnosis or final image
interpretation nor be stored for retention purposes.
Only use original (uncompressed) or lossless compressed images for diagnostic purposes and
activate either “Server mode (lossless)” or “Client mode” in the iQ-4VIEW configuration to
ensure that the images are displayed and processed in FDA approved diagnostic quality.

WARNING:
Danger of unauthorized access to patient information.
iQ-4VIEW works with image data containing information that identifies individual patients
(while displaying and processing data). It is the user’s responsibility to prevent unauthorized
access to the information to ensure the patient’s rights to data protection.
Users and log in mechanisms should be used to regulate access to the device/system on which
iQ-4VIEW is used.

Any further warnings or precautions regarding individual functions of iQ-4VIEW are documented
in the respective sections of the iQ-4VIEW Instructions for Use and this Administration Guide.
NOTICE:
The system requirements stated below are the requirements and recommendations valid at the
release of this software version and/or the release date of this document.
This information is subject to change over the course of the product’s life-cycle. The manu-
facturer will inform about updates regarding the system requirements when iQ-4VIEW
becomes available for systems other than those defined below (e.g. newer operating systems).
Contact your reseller or the manufacturer for the latest information.
Keep in mind that iQ-4VIEW as a medical device will not automatically support any new
technology that becomes available on the market. The software will have to be tested
according to legal regulations before being released for such systems.
For the iQ-4VIEW server the system requirements concerning both hardware and software
are:
For the iQ-WEB server-side hardware and software requirements refer to the system
requirements of the main application iQ-WEB. For further details regarding the specifications,
refer to the iQ-WEB user documentation of the respective software version.
For the iQ-4VIEW client the system requirements concerning both hardware and software are:
If using third-party applications on the same system, higher requirements may apply.
It is recommended that the application is used only within a secured environment as iQ-4VIEW
does not support any specific security measures. A secured environment includes at a minimum:
▪ Firewall or router protections to ensure that only approved external hosts have network
access to the computer, on which iQ-4VIEW is used.
▪ Firewall or router protections to ensure that the computer, on which iQ-4VIEW is used, only
has network access to approved external hosts and services.
▪ Any communication with external hosts and services outside the locally secured environment
uses appropriate secure network channels (e.g. such as a Virtual Private Network (VPN)).
▪ A regularly updated anti-virus/malware software.
We recommend the use of up-to-date anti-virus software on the computer on which iQ-4VIEW
is run. The virus definitions must be updated regularly (they should not be older than 2 weeks).
Other network security procedures such as automated intrusion detection may be appropriate in
some environments. Additional security features may be established by the local security policy
and are beyond the scope of these instructions.
The iQ-4VIEW software component is part of the iQ-WEBX installation package. To use
iQ-4VIEW, the iQ-WEB software and all necessary third-party software (MySQL and Apache/PHP)
must first be installed on the PACS server. Refer to the iQ-WEB Administration Guide document
of the appropriate software version for all details on how to install iQ-WEB.
Only after this general installation was successful it is possible to run iQ-4VIEW. Since iQ-4VIEW
is a zero footprint web application there are no further installation processes on client devices.
To open iQ-4VIEW in a browser, it is necessary to get access to the studies stored in iQ-WEB.
For further instructions on how to open iQ-4VIEW and load studies into the viewer, refer to the
iQ-4VIEW Instructions for Use. These instructions is part of the installation package and can be
accessed by:
▪ Opening Windows Explorer on the server, navigating to the “doc” folder of the iQ-WEBX
installation (e.g.: C:\Program Files\iQ-WEBX\PACS\php\iQ-4VIEW\server\doc) and opening
“[Link]”.
▪ Opening the document from within the application using the “user documentation” button (
).
▪ Downloading the document from the manufacturer’s website at [Link]
[Link]/filebase/.
NOTICE:
Adobe Acrobat Reader or another PDF reader must be installed/available on the system to
open and view the iQ-4VIEW instructions for use.
From a technical point of view, an upgrade from an existing iQ-4VIEW installation to a newer
software version is very easy.
Simply run the new iQ-4VIEW installation file and install the new software version, which is
included in the iQ-WEBX installation package, on top of the existing installation. All configuration
files as well as all image data will be maintained during the upgrade. Follow the instructions
given for iQ-WEB to ensure that the new version of iQ-4VIEW is placed correctly onto the PACS
server.
Keep in mind that a license valid for one software version will not be valid for a newer software
version. That means, after the upgrade you will be able to use the new version for the default
30 day trial period. Afterwards, you will need a new full license to run the new version.
Since iQ-4VIEW does not deploy any persistent data to the client devices, there is no way and,
first of all, no need for an uninstallation procedure.
For a complete removal of the server components it is necessary to remove the iQ-WEBX
package. If you wish to uninstall iQ-WEBX completely from your system, refer to the iQ-WEBX
Administration Guide of the respective software version.
Also refer to the application’s End User License Agreement to learn which permissions and
restrictions come with the respective license type.
NOTICE:
All time-limited licenses are date-sensitive. Intended or involuntary modification of the
Windows system date is handled as an attempt to tamper with the license and will invalidate a
time-limited license. As a result the application can no longer be accessed. In case of date
corruption, contact your local reseller. Keep in mind that the issuing of a replacement key
might only be possible for a fee.
After installing the software, the application runs by default as an evaluation version with a
license that is limited in time and maximum number of concurrent users.
After 30 days the application stops working unless a full license is obtained and the software is
activated with a specifically created activated key. The activation is done on the server on which
iQ-WEBX is installed. The individual iQ-4VIEW clients must not be licensed separately.
The following instructions only concern the activation of the iQ-4VIEW software license. The
activation is done in the web interface of iQ-WEB:
The key will be stored on the iQ-WEBX server PC and does not have to be entered each time you
start the application. Hardware modifications are still possible without invalidation of the license
(max. three components), but the hardware fingerprint will change.
NOTICE:
Do not press “Uninstall License” unless you really wish to reset your license. During this
process, the hardware fingerprint will change and your previous activation data will become
invalid. You will have to request a replacement key, which might require a fee.
The purpose of this chapter is to provide the administrator with all steps necessary to do either
of the following actions:
▪ Change an existing software license on the system (e.g. renewing a time-limited license)
▪ Removing an existing software license from a system
▪ Move an existing software license from one system to another
In some cases it may become necessary to change an existing license, i.e. to enter new
activation data even though a license is currently active. This may happen if:
▪ You wish to renew a time-limited license (e.g. trial) that is about to expire
▪ You wish to upgrade from a time-limited license to a time unlimited license
▪ You wish to apply a license that allows more concurrent user accesses
After you have received the new license activation data from your local reseller (registration
name and activation key), log into the iQ-WEB interface with user “root”, go to the “Tools” and
select the “Licensing” tab. Then enter the new registration name and activation key in the “Install
License” section and click the “Install License” button to confirm the activation.
NOTICE:
Contact your local reseller for the procedures of changing an existing license.
When a license is removed from your current system, it will no longer be valid. The hardware
fingerprint will change and the previously used activation key becomes invalid. This procedure
might be necessary if you want to migrate an existing license to a different computer or if you
do not wish to use the software any longer. In those cases, you have to send the uninstall
information file (created while the license is removed) to your local reseller.
If you wish to remove your purchased license, you need to follow the instructions given here:
A license migration of iQ-4VIEW is only possible in from one iQ-WEB to another, since iQ-4VIEW
will only function in combination with a licensed iQ-WEB installation.
If you want to move your purchased iQ-4VIEW license from one computer to another you need
to follow the instructions given here:
NOTICE:

Keep in mind that for replacement keys a fee might be charged. Therefore, contact your
reseller BEFORE you transfer your iQ-4VIEW license to a new computer!
The software itself needs no maintenance. However, the capabilities and the performance of the
software can be affected by limitations and failures caused by the hardware, the network or
other software installed on the system. Therefore, the proper operation and the maintenance of
the hardware and network are necessary (e.g. professional connection to power and network,
sufficient ventilation, regular cleaning of the fan etc.). Instructions regarding hardware and
software specifications as well as software maintenance given by the manufacturers must be
adhered to.
The user should always use the latest software version and update existing installations when
updates become available.

All modifications to the medical software have to be made by IT service personnel. This
includes the installation, verification as well as changes in the software. The risk of malfunction
is relatively high when modifying software. Abnormal termination of the software as well as
temporary or permanent data losses are possible when improperly administered. Therefore all
modifications to the software are performed solely by service personnel.
Modifying application folders and or files to different locations, deleting or renaming them
without considering other parts may cause problems in the functioning of the software. Keep
the file and folder structure intact and only follow the user documentations’ instruction for
configuring the application.

In order to ensure the faultless operation of iQ-4VIEW, it is necessary to properly keep up the
system by performing specific recommended maintenance tasks.
The following table provides you with an overview of all recommended maintenance activities
and states when and how often you should schedule them. You will find all details for every task
in the section stated under “Reference”.
As a software application running in a web browser, iQ-4VIEW is intended for frequent, but not
for continuous use.
On the clients, on which iQ-4VIEW is used, the web browser should be restarted at least once a
day. And the client device must be shut down and restarted regularly, preferably daily, but at
least once a week.
The server component of the iQ-4VIEW software will usually be installed on a Windows server
system together with iQ-WEB. The system must be shut down and restarted at least once a
month. This automatically also restarts the software and ensures the freeing of potentially
blocked system resources.
In cases where the server component is installed on a client computer (e.g. with Windows 10
OS), the software and the system must be shut down and restarted at least once a week.
To adhere to the laws and regulations concerning image reading devices it is necessary to
calibrate and regularly recalibrate the displays and monitors of the iQ-4VIEW workstations that
are used for diagnostic purposes.
This also includes the following regular constancy tests of all diagnostic displays:
▪ On every working day, visually check the overall image quality (using test image TG18-OIQ).
▪ Half-yearly, visually check the homogeneity of luminance as well as color impression and
color constancy (using test image TG18-UN80).
▪ Half-yearly, perform a constancy test according to DIN 6868-157 with appropriate
measurement devices (luminance meter and illuminance meter).
When the constancy check fails, a recalibration of the respective diagnostic display must be
performed.
WARNING:
Danger of misdiagnosis due to use of non-calibrated displays.
If the application is installed for diagnostic purposes the displays at the workstation need to be
checked and, if necessary, (re)calibrated according to DIN 6868-157 to guarantee the
diagnostic quality of the displays. The quality checks must be performed using the appropriate
test dataset(s). The person doing these checks must make sure that the measuring device has
a valid calibration status.

Computer viruses and malware hold a considerable risk. A virus/malware infection may
potentially lead to data losses and to data inconsistencies. To avoid the risk of a virus infection
of the computer, on which the software is running, all systems should be furnished with anti-
virus software that needs to be updated regularly. Updates should be run as soon as they become
available, but at least once a day.
Anti-virus/anti-malware checks should also be run regularly on the system where the iQ-4VIEW
server component is installed (together with iQ-WEB) and on all devices on which it is used as
client. It is recommended to run a check after virus definition updates.

computer, on which iQ-4VIEW is used, all systems should be furnished with anti-virus software
that needs to be updated regularly.

A failure or partial loss of the network/internet connection or a low bandwidth within the medical
network can pose risks when imaging data cannot be made available at the receiving devices, is
received only incompletely or is not transmitted fast enough, especially in emergency cases.
Therefore, check daily all network/internet connections between the iQ-4VIEW clients and the
iQ-WEB server for their availability and performance, especially in teleradiology scenarios where
the client and the iQ-WEB server are not located in the same network (WAN). This check should
be done before the first procedure is scheduled at the respective receiving iQ-4VIEW client.
Example: If the night shift starts at 8 p.m. and the radiologist is supposed to load imaging data
from iQ-WEB for emergency reading, verify the connection at the beginning of the night shift.
Where emergency cases are supposed to be read at the receiving iQ-4VIEW client, the access
time to the images must be no longer than 5 minutes.
You should check regularly if your system is still up-to-date and perform necessary updates.
Check the following items:
▪ Install security updates and patches for the operating systems on the server and clients
within one week after their release.
▪ Install security updates and patches of the web browsers used on the clients within one week
after their release.
▪ Check quarterly for new system drivers but update them only after checking back with your
local reseller to ensure the compatibility with the currently installed iQ-4VIEW version.
▪ Check quarterly if software patches for the currently installed iQ-4VIEW software are
available. If yes, update the system accordingly.
▪ Check quarterly if a new software version of iQ-4VIEW is available. Verify that the software
requirements of the new version match the system. Contact your local reseller to discuss
updating your system to the new version.
▪ Check if the used Windows operating system on the server and the operating systems on the
clients (Windows, Android, iOS) are still supported by their manufacturer. Check how long

We recommend to record and count the number at least of severe software errors, such as
crashes, throughout the entire software life-cycle and to report such incidents to your local
reseller or directly to the manufacturer.
▪ An up-to-date server system (e.g. Windows Server 2019) with all necessary security patches
and current anti-virus/malware software should run 30 days without crashing.
▪ An up-to-date client system (e.g. Windows 10) with all necessary security patches and
current anti-virus/malware software should run 1 week without crashing.

The iQ-4VIEW user documentation consists of the instructions for use and this administration
guide.
The documentation can be accessed from every client device directly from the upper toolbar of
iQ-4VIEW. Open the iQ-4VIEW viewer and click the appropriate icon in the upper toolbar:

There are two options for iQ-4VIEW that are user independent. These settings can be accessed
by navigating to the SETTINGS → System page in the iQ-WEB user interface. In the middle of
the page is the section for iQ-4VIEW relevant settings.
iQ-4VIEW will terminate an active user session after a defined amount of minutes of user
inactivity to prevent unintended occupation of concurrent user slots. By default this value is set
30 minutes after the last action of the user and can be changed to any number except 0.
This option enables/disables the automatic pre-generation of the iQ-4VIEW web images
whenever new DICOM data is added to the iQ-WEB archive. This option is enabled by default
and will create lossless (PNG) images of the DICOM data. Additionally, JSON files are generated
holding important information from every DICOM object.
Furthermore, a value in days can be defined to automatically purge these images after a certain
amount of time. A value of 0 will deactivate this feature.
The image generation can be additionally defined with the following options.
iQ-4VIEW allows a user to create structured reports, to store draft versions in a special database,
as well as to create, delete or edit templates that help to save time when inserting often-used
phrases or report fragments. The final report will be created on the iQ-WEB server as a DICOM
Structured Report (SR) and will be added as a new series to the study for which the report was
created.
However, a user must be granted permission to access this function by the PACS administrator
who handles the iQ-WEB user management.
Log into the iQ-WEB web interface with an administrator account and access the user
management under “SETTINGS” → “Users”. Select the user you wish to grant permission and
click the “Edit” link. In section “General Privilege Settings” enable the checkbox “Structured
Reports Privilege” and then scroll to the end of the page and click “Modify User” in order to
apply the changes.
iQ-4VIEW allows a user to create secondary capture images either from an original image loaded
into the viewer in order to permanently store changes such as measurements or annotations
performed in that image or by importing a JPEG file from an outside source (e.g. local directory,
removable media or camera for mobile devices).
However, a user must be granted permission to access this function by the PACS administrator
who handles the iQ-WEB user management.

It is possible to configure which information shall be displayed in the text overlay of the DICOM
images. This can be done by using the text overlay configuration, accessed by the respective
sub-menu entry from the overlay button.
Opened for the first time, the text overlay configuration dialog shows the default settings. By
clicking the “Create custom overlay” button it is possible to create a custom text overlay
template.
The custom configuration is done in dependence of the SOP class. It is either possible to bind
the custom template to a specific SOP class or to all SOP classes. The SOP class drop-down list
offers all SOP classes specified in the DICOM standard.
Custom template
As the newly created template bases on the default settings, all settings are taken over from the
default one.

After that all defined text entries are display below the template set. The template set is a
collection of all text elements defined for the current position. Each element in the template set
will be shown in the text overlay in a separate row. By selecting a specific row the content will
be shown in the bottom edit box. In conclusion, the template set shows tags of annotations that
will be shown at a specific position in the image processing area, when the user loads an instance
with the selected SOP class.
If a new text row should be added, click the “Add” button ( ). Go to the edit box below and
type in the text to be displayed in the overlay. Besides adding simple text, DICOM tags can also
be added. iQ-4VIEW parses these DICOM tags and replaces them with the respective DICOM
information. To properly parse the DICOM tags they have to follow a specific format.
If an existing row should be deleted, select the specific row and click the “Delete” button ( ).
If a specific text overlay label needs to be modified, just click into the referring row and edit the
label in the bottom edit box. In addition it is possible to change the row position of a created
It is also possible to highlight specific text elements by using html codes. Typical codes are found
in the table below.
Add one or multiple code sequences to the edit box. The sequences have to surround the text
that needs to be highlighted.
The custom overlay is saved by using the “Save” button in the upper left corner of the
text overlay configuration dialog. If the saving process is done the changes will be
applied immediately.
If a custom overlay was created the text overlay configuration dialog changes and two new
buttons replace the “Create custom overlay” button.
▪ “Delete & use default”: The custom overlay will be deleted and all settings will be restored to
their default values.
▪ “Defaults for current SOP”: It is possible to reset a specific SOP class configuration to its
default values. Maybe this comes to place if a custom overlay was created for all SOP classes
and a specific SOP class needs to be configured in another way.
In the viewer’s “Settings” dialog, there are two options that affect the structured
reporting.
This second option handles the usage of a specific character set for the creation of the structured
report DICOM object. This attribute is responsible for the interpretation of the character sets and
how the letters/characters are displayed in the application.
By default, the option “Same as original dataset” is enabled. This option uses the same character
set for the creation of a structured report as is used for the study that the report will be appended
to after complete verification. If, however, the report is written in a language that was not used
for the creation of the images, then it might be necessary to select a specific character set from
the list to ensure that the structured report can be saved properly. This may be the case when
the patient information in the images is encoded in Latin letters/characters but the report is
written in Russian, Japanese or another language not using Latin characters.

SOP classes:
The product iQ-4VIEW implements no DICOM services to transfer DICOM datasets (files) via
network transmission, to query other DICOM systems for workflow and data information or to
send print requests to DICOM Print aware systems (printer).
However, the iQ-4VIEW DICOM viewer supports the display and processing of various image and
non-image SOP classes. The following list states all SOP classes that are currently supported:

for further details on what iQ-WEB supports.
