INTRODUCTION

NARAYANA HRUDAYALAYA
Narayana Health (formerly known as Narayana Hrudyalaya was established in 2000 by Dr. Devi Shetty,
a cardiac surgeon, in Bangalore, India. The hospital was founded with the mission of providing high-
quality, affordable healthcare to the masses.
The first hospital was a 280-bed heart hospital, and it quickly became a success. In the following years,
Narayana Health expanded rapidly, opening hospitals and heart counterscarps India. The group now has
over 5,859 operational beds in 21 hospitals, 6 heart centers, and 19 primary care facilities across India.
The world perceives us as a low-cost Indian healthcare service provider; what we are engaged in is a
passionate journey to establish ourselves as the lowest-cost, high-quality healthcare service provider in
the world.
At Narayana Health, we are convinced that 'quality' and 'lowest cost' are not mutually exclusive when it
comes to healthcare delivery. In fact, we are well on our way to demonstrate that we are not running
our institution as just another number-only business, but are attractively placed to create an affordable,
globally-benchmarked quality-driven healthcare services model.
At Narayana Health, we will continue to trust what has worked for us in the past. We will keep our model
predominantly asset-light; we will reinvest our accruals; we will engage with governments for
2
land and help them achieve affordable healthcare promises made to their constituencies; we will rent
premises from private owners and create small-to-large clinics within existing hospitals. As a result of
this flexible approach, we will commission hospitals, medical facilities, and clinics across the breadth
of this country, getting closer to patients and taking the promise of quality affordable healthcare to the
doorstep of the country's millions.
It is this exciting prospect that stokes my motivation to do bigger and better for the glory of the country
that is mine.
Narayana Health is headquartered in Bengaluru, India, and operates a network of hospitals across the
country, with a particularly strong presence in the southern state of Karnataka and eastern India, as well
as an emerging presence in northern, western and central India. Our first facility was established in
Bengaluru with approximately 225 operational beds and we have since grown to 21 Hospitals + 1 Cayman
Islands and 6 heart centres, 19 primary care facilities across India and an international hospital in the
Cayman Islands. The group now features over 5,859 operational beds through a combination of greenfield
projects and acquisitions. We believe that the "Narayana Health" brand is strongly associated with our
mission to deliver high-quality, affordable healthcare services to the broader population by leveraging
our economies of scale, skilled doctors, and an efficient business model.
In aggregate, our centres provide advanced levels of care in over 30 specialties, including Cardiology and
Cardiac Surgery, Cancer Care, Neurology and Neurosurgery, Orthopaedics, Nephrology and Urology,
and Gastroenterology.
Narayana Health is known for its use of innovative technologies and its focus on patient care. The group
has been at the forefront of several medical advancements, including robotic surgery and minimally
invasive cardiac surgery. Narayana Health is also one of the largest providers of organ transplantation in
India.
BACKGROUND
Dr. Shetty, like many doctors of his era, went to England after graduating in 1982 from Kasturba Medical
College in the coastal town of Mangalore in Karnataka, India. He worked at a hospital in Midlands,
England and later at the Guys Hospital in London. But after a decade of experience abroad, he returned
3
to India and founded the Asian Heart Foundation (AHF) in Calcutta in 1989 as a non-profit
foundation focused on cardiac care. AHF initially helped set up hospital for other organizations. The first
one was opened in 1989 by the B.M. Birla Heart Research Institute. In 1997, Dr. Shetty assisted in setting
up the Manipal Heart Foundation’s hospital in Bangalore. Dr. Shetty’s AHF, in association with the West
Bengal (an eastern Indian state) government, built a 150-bed cardiac facility in Calcutta that quickly
became the largest cardiac hospital in eastern India. In 1999, AHF also built its own cardiac hospital
called the Rabindranath Tagore International Institute of Cardiac Sciences (RTIICS) in Kolkata (formerly
Calcutta).
Dr. Shetty never had problems raising the funds to build the hospital. A stockbroker patient donated
£72,000 (US$142,732) as working capital, while a private company, Tata Finance, provided soft loans
for buying equipment. Another patient’s son, who had a construction company, offered to build a cardiac
diagnostic laboratory, and the Armenian Church, impressed by Shetty’s venture, came forward with a
pledge of US$1 million. “Believe it or not, it’s not difficult to arrange for funds when your causeis noble,”
opined Shetty in an interview to the New Scientist magazine in 200213. While Dr. Shetty was in Calcutta
working at RTIICS, he also served as Mother Teresa’s personal cardiac surgeon, and this association
deeply inspired him to think deeply about how he could better serve the poor. In a letter he wrote to
children on completing 4,000 pediatric surgeries, Shetty shared what the nun had told him at the ICU14
of the hospital where she was convalescing: “Now I know why you are here. To relieve the agony of
children with heart disease. God sent you to this world to fix it.” Shetty wrote, perhaps the best
compliment that I have ever received.” 15 Demand for cardiac surgeries in India was underserved. In
2003, it was estimated that there were only 55,000 open-heart surgeries that were performed in India per
million people whereas the demand was upwards of 125,00016.
The cost also remained high and varied from hospital to hospital, depending on the type of facility. The
top-tier private hospitals typically charged anywhere from US$1,500 to $6,000 for an open-heart
surgery. Some of the government hospitals, such as the All-India Institute of Medical Sciences in Delhi
(AIIMS), charged much less (US$1,200) but did not have adequate capacity to treat a large number of
patients. In 2001, Dr. Shetty’s father-in-law Narayan Shetty, who owned a construction firm Shankar
Narayana Constructions (SNC) that was celebrating its 50th anniversary, invited him to start a state-of-
the-art cardiac hospital in Bangalore. SNC provided 25 acres of land on a 13 “Interview: India’s Heart-

Healthcare for All: Narayana Hrudayalaya, Bangalore 4 30-year lease to Dr. Shetty’s NH Private Limited
and provided the initial capital to build and operate the hospital. Dr. Shetty’s family also stood as
guarantors for the loans that NH needed to raise to equip the facility. The hospital was named Narayana
Hrudayalaya17 with the following mission statement: “We at Narayana Hrudayalaya Foundation,
Bangalore and the Asia Heart Foundation, Kolkata have a dream. A dream of making sophisticated
healthcare available to the masses, especially in a developing country like our own.” In order to support
its mission to serve the poor, NH created a business model that, in its core, operated on the principle that
high quality care would attract full-price paying patients, and along with NH’s foundation, these patients
would subsidize surgeries performed for the poor. NH was modeled as a business enterprise as opposed
to AHF that was a non-profit foundation. Dr. Shetty realized thelimitations of completely charity-based
models that were incorporated as charitable trusts. As Dr.Shetty put it, “We realize that a trust cannot
grow and become huge since only way a trust can raise money is by donations and loans and both models
will not scale up to the size we are looking at. So we are effectively using both models to suite the current
legal requirement of the country.” 18 Traditionally, leading hospitals in the world had steadily
transformed into academic and training institutions in order to pursue excellence. However, in India,
private companies were not permitted to start educational institutions. Therefore, Dr. Shetty also started
a third foundation called Hrudayalaya Foundation and Narayana Hrudayalaya charitable trust
(HFNHCT). Dr. Shetty explained, we started foundation, which is essentially a trust, which can run
various educational programs.”19 From an organizational perspective, Dr. Shetty’s family owned all the
three entities: AHF, NH, and HFNHCT, and Dr. Shetty was the head of all three. In the initial part of the
first phase, in 2001, NH had 225 beds. That figure has increased by 100 beds each year until the hospital
reached a built-up area of 250,000 square feet of stateof-the-art facilities with ten state-of-the-art
operating theaters and 500 beds in 2005 and 2006 with a capacity to perform up to 25 surgeries daily. NH
was also working on a second phase that began later in 2005 and is slated to end in 2008. Shetty’s long-
term staff member, Vasuki, a long- standing associate of Dr. Shetty and NH’s communications head,
recalled the grand opening day in 2001: “We were very skeptical on the day of inauguration. We had
distributed 500 invitations and ordered 300 chairs to seat the guests. Some of us felt that the hospital
being several miles from the center of the city may not be able to attract people for the function and
later for the treatment as well
5
But, Dr. Shetty was sure. They will come, he said. In his opinion, the hospital was for people not just
from Bangalore but any poor person from across the country. His point was if someone traveled a couple
of thousand kilometers to come to Bangalore, they won’t mind the additional 20 kilometer’s commute to
the hospital. 17 In the ancient language of Sanskrit, “Hruday” meant heart and” alaya” referred to temple

decreasingsignificance of the dealer network Narayana Hrudayalaya has to build a new robust supply
chain network. That can be extremely expensive.
• Low investments into Narayana Hrudayalaya's customer-oriented services - This can lead to
competitors gaining advantage in near future. Narayana Hrudayalaya needs to increase investment
into research and development especially in customer services-oriented applications.
• High turnover of employees at the lower levels is also a concern for the Narayana Hrudayalaya . It
can lead to higher salaries to maintain the talent within the firm.
• Loyalty among suppliers is low - Given the history of Narayana Hrudayalaya coming up with new
innovations to drive down prices in the supply chain.
• Declining market share of Narayana Hrudayalaya with increasing revenues - the Healthcare
Facilities industry is growing faster than the company. In such a scenario Narayana Hrudayalaya has
to carefully analyze the various trends within the Healthcare sector and figure out what it needs to
do to drive future growth.
• Declining per unit revenue for Narayana Hrudayalaya - competitiveness in the Healthcare
Facilities industry is putting downward pressure on the profitability. A starting guide to manage this
situation for company name is – objectively assessing the present value propositions of the various
products.
OPPORTUNITIES
Opportunities are potential areas where the firm chan identify potential for - growth, profits, and
marketshare.
• Trend of customers migrating to higher end products - It represents great opportunity for
Narayana Hrudayalaya, as the firm has strong brand recognition in the premium segment, customers
have experience with excellent customer services provided by Narayana Hrudayalaya brands in
12
the lower segment. It can be a win-win for the company and provides an opportunity to increase the

after initial success of a new product.
THREATS
Threats are factors that can be potential dangers to the firm's business models because of changes in
macro-economic factors and changing consumer perceptions. Threats can be managed but not
controlled.
• Competitors catching up with the product development - Even though at present the Narayana
Hrudayalaya is still leader in product innovation in the Healthcare Facilities segment. It is facing
stiffchallenges from international and local competitors.
13
• Shortage of skilled human resources - Given the high turnover of employees and increasing
dependence on innovative solution, companyname can face skilled human resources challenges
in thenear future.
• Trade Relation between US and China can affect Narayana Hrudayalaya growth plans - This
can lead to full scale trade war which can hamper the potential of Narayana Hrudayalaya to
expand operations in China.
• Distrust of institutions and increasing threat of legal actions for Narayana Hrudayalaya - As the
WTO regulations and laws are difficult to enforce in various markets. Legal procedures have
becomeexpensive and long drawn process. It can lead to less investment into emerging markets
by NarayanaHrudayalaya thus resulting in slower growth.
• Growing technological expertise of local players in the export market - One of the biggest threat
of tie-up with the local players in the export market for Narayana Hrudayalaya is threat of losing
IPR. Theintellectual property rights framework is not very strong in emerging markets especially in
China.
• Saturation in urban market and stagnation in the rural markets - For Narayana Hrudayalaya
thistrend is an ongoing challenge in the Healthcare Facilities segment. One of the reasons is that
the adoption of products is slow in rural market. Secondly it is more costly for Narayana
Hrudayalaya to serve the rural customers than urban customers given the vast distances and lack of
infrastructure.

MARKETING DEPARTMENT
Marketing is the process of planning and executing the conception, pricing, promotion, and
distribution (4Ps) of ideas, goods and services to create exchanges (with customers) that satisfy
individual and organizational objectives. Marketing plays an important and pervasive role in
the healthcare marketplace. Till 1980, the concept was alien to healthcare. Today, one can
witness a great deal of marketing taking place in all HealthCare organizations. It is now very
common to see that virtually every hospital place ad in newspapers and magazines to tout its
facilities and services. Hospital salaried physicians give talk shows in TVs, hold camps, deliver
popular health related lectures. Marketing takes place when at least one party to a potential.
The marketing department exhibits the marketing activities in two different channels – external
and internal channels. The external marketing channels is making the public aware or even
makingthe patients aware of all the treatments that they can avail from the hospital. These news
reaches them mainly through online applications such as Facebookpage, Instagram page,
YouTube channels and LinkedIn pages. There are also different healthcare magazines that
advertise about the services that are provides. On considering the internal marketing aspects,
these are done so asto make the employees well aware about the infrastructure and all the
possible services thatthe organization provides so that they, their family are aware as well as
they can pass the information to others
In conclusion, the marketing department within a hospital plays a pivotal role in promoting
healthcare services, fostering community engagement, and ensuring the hospital's growth and
success. Through strategic campaigns, patient outreach initiatives, and effective
communication, the marketing department not only raises awareness about the hospital's
offerings but also cultivates trust and credibility within the community. In an era of increasing
competition and evolving patient preferences, a well-functioning marketing department is
essential for ensuring that healthcare institutions can effectively meet the needsof their patients
and maintain a strong presence in the healthcare industry.
