Carrier Agreement
1. Parties
This Carrier Agreement ("Agreement") is entered into between Big Box Trucks/Utah
Valley Express LLC, a companies organized and existing under the laws of FMCSA, with
its principal place of business located at 3787 N Prairie grass Dr Lehi UT 84043
("Carrier"), and (“Contractor or Self Employed”), a company or individual (“Self
Employed”), organized and existing under the laws of the state of Utah and the FMCSA.
2. Services Provided
Contractors or independent business owners(Self employed), agrees to provide
transportation services for the following goods:
● Provide delivery services(“Appliances, Fitness Equipment, Furniture”) with the
highest quality of service to the customer, which may or may not include:
installations, relocates, pickups, white glove.
3. Pricing and Payment
The pricing for the transportation services shall be as follows:
1 Route $140.00 Helper
1 Route $ 180.00* Driver
● Each contractor or self-employed will be paid using Form 1099, and no value
related to their taxes will be withheld.

* The services to be paid to contractors or individuals will be based on route and
not on an hourly basis. This means that payment ranges vary according to
location, date, quantity, and performance.

● Payment will be made weekly via direct deposit (ACH). Each contractor must sign
a direct deposit authorization to their bank account.
4. Term and Termination
This Agreement shall commence on 04/01/2024 and shall continue until terminated by
either party upon [Notice Period] written notice.
● A bond will be activated according to the rules and regulations of the Department
of Transportation, or FMCSA for short. This bond will be administered by
("Carrier"), and it will be deducted from the Contractors or individuals and could
be on a weekly or monthly basis as decided by the general administration
("Carrier"), and it will be executed annually. The refund of this bond will be based
on the total accumulation of damages either by equipment or damages to third
parties after 45 working days from the termination of the contract, with the
purpose of allowing the general administration ("Carrier") to continue operating
and avoiding an economic breakdown due to damages caused during delivery
and installation services.
● All Contractors or self-employed individuals must follow and comply with the
rules, conduct, and values of the various clients and respect the values of those
companies as well as those of the general administration ("Carrier").
5. Liability and Insurance
Each Contractor or Self-employed individual must obtain insurance to cover expenses
for any type of loss or damage. Failure to comply with this requirement may increase
the bond and liability of the contractors and self-employed.

6. Compliance and Regulations
Both parties agree to comply with all applicable laws, regulations, and industry
standards related to the transportation services.
● All contractors who are drivers must have their DOT medical card and a clean
driving record.
● All contractors or self-employed individuals must wear the appropriate uniform
and comply with the standards of the clients as well as maintain good conduct.
7. Confidentiality and Privacy
Both parties agree to maintain the confidentiality of any proprietary or confidential
information shared during the course of this Agreement.
8. Dispute Resolution
Any disputes arising under this Agreement shall be resolved through mediation, with
litigation as a last resort. The laws of [Your Jurisdiction] shall govern this Agreement.
9. Miscellaneous Provisions
● This Agreement constitutes the entire agreement between the parties and
supersedes any prior agreements or understandings.
● Amendments to this Agreement must be made in writing and signed by both
parties.
● Neither party shall assign this Agreement without the prior written consent of the
other party.
10. Signatures
This Agreement shall be executed in duplicate, with each party retaining one original
copy.
