2- Purpose
The purpose of Work Authorization Program (WAP) is to manage risks and define a formal
safe system of controlling work or activities performed in restricted or hazardous areas
during SAWCOD operations. This program establishes the minimum requirements and
defines responsibilities for issuing /receiving Work Authorizations, and for outlining the
competencies required for those responsible for implementing this system. When issued,
WAP serves as a written conditions and requirements agreed upon by the Issuer and
Receiver. WAP shall also ensure that certified and competent person have thought about
foreseeable risks and those risks are avoided using the suitable precautions so that the
job can be carried out safely.

3- Scope
This WAP is applicable to all Southern Area Well Completion Operations Department
(SAWCOD) operations performed by contractor (Service Companies) or Saudi Aramco
personnel belongs to SAWCOD units.

4- Definitions
4.1. Confined Space: any space that
 Has limited or restricted means of entry and exit
 Is not designed for human occupancy
 Contains or has the potential to contain a hazardous atmosphere
 Contains any other recognized serious health, safety or environmental
hazard(s)

4.2. Confined Space Entry:


The entrance of any part of the body into a space that meets the criteria for a
confined space. Entry includes all periods of time when the confined space is
occupied.

4.3. Confined Space Entry Standby Man:


Individual assigned by the WAP Issuer at the designated entry point to continuously
monitor the confined space entry while personnel (entrants) are inside the confined

space. He shall be trained and qualified to carry out his responsibilities as per the
Saudi Aramco Construction Safety Manual CSM (I-6 Confined Spaces) and this Safety
Instruction Manual (SIM).

4.4. Confined Space Entry Supervisor:


A WAP issuer, or an individual who has been assigned by an issuer, who is responsible
for directing all aspects of the confined space entry. Confined space entry
supervisors shall be trained and qualified to carry out his responsibilities as per the
Saudi Aramco Construction Safety Manual (CSM) and this Safety Instruction Manual.

4.5. Emergency Work:


An activity undertaken during a declared emergency at SAWCOD operational well
site(s) under the direction of the Incident Commander.

4.6. Hazard Analysis:


An assessment of work activity conducted during the joint site inspection before the
work is started. It is performed by the WAP issuer and the receiver. For this purpose,
the work steps, the work equipment to be used and the working environment are
discussed at the job site. The preventive measures specified by the applicable
WAP(s) are reviewed to verify they are in place and address the hazards of the work
activity.

4.7. Hot Work:


Any activity that may develop sparks, flames or heat sufficient to cause ignition.
Examples include but not limited to internal combustion engines in restricted areas,
welding, torch usage, grinding, abrasive blasting, and work on or in close proximity
to live electrical apparatus, and the use of a device not rated for the electrical
classification of the area.

4.8. Joint Site Inspection:


An inspection conducted by the WAP issuer and the receiver to conduct a well
location hazard analysis, perform necessary gas tests and review the work
authorization conditions.

4.9. Line Breaking


An activity associated with the initial opening (first break) of equipment or piping
that is part of a system that contains (or has the potential to contain) flammable,
combustible, toxic or harmful materials e.g., lubricators, CT strippers, Well Head
Isolation Tool (WHIT), between the wellhead and upstream choke, ball catcher,
upstream side connections, blind flanges from the tree, and TCA or CCA lines etc.

4.10. Restricted Activity:


A well completion activity/task that is taking place on well location and attended by
SAWCOD operational sites and present potential hazards to personnel, facilities,
equipment or operations.

4.11. Restricted Area:


An area where hydrocarbons, flammable liquids or gases, and toxic agents are
handled, stored, piped or processed in bulk quantities including oil and gas wells.

4.12. Watchman:
An individual designated by the issuer when needed or as required during major
operation e.g. Frac and Coiled Tubing, and identified on the WAP form. He should
be familiar with the work activities and aware of the inherent hazards and
knowledgeable in the controls required to ensure that job is performed safely. He is
required to be familiar with the procedures for notifying appropriate personnel in
the event of any abnormalities during the performance of the job.

4.13. WAP –Work Authorization Program:


Is an internal administrative control developed by SAWCOD to add safety protective
layers on SAWCOD operations sites and site less for activities performed by
contractors or other support organization to ensure proper initiation, review,
approval, execution and close out of all work authorizations.

4.14. WAP Issuer


Is SAWCOD Person In Charge (PIC) on location trained and certified to issue, perform
joint site inspection, hazard analysis, gas testing and close WAP.

4.15. WAP Receivers


A contractor senior person (engineer, supervisor or a leader) who are holding a
supervisory position on the segment they are working in and aware of all related
hazards, associated risks and mitigation controls.

Note: for BPV jobs, the receiver will be SAWCOD BPV unit supervisor or group leader trained
and certified as WAP receiver.

4.16. Work Crew


A group of workers who report to a common supervisor (e.g., foreman, supervisor or
group leader). Crew workers may have different craft skills. However, they are
performing activities under the direction of a single supervisor.

5- WAP Functions
Functions of WAP can be summarized as follows:

5.1. WAP must be issued, as appropriate for specific work in a specific site.
5.2. Ensure proper authorization of designated work.
5.3. Clear understanding of the specific work, hazards involved, limitations and
duration of the job to be carried out.
5.4. Specify precautions to be taken prior to starting the job.
5.5. Provide not only a system of continuous control, but also continuous
monitoring and supervision is carried out by a competent person.
5.6. Provide control of work activities that may interact or affect one another.
5.7. Provide a formal handover procedure when a WAP is issued for a period longer
than one shift.

6- Responsibilities
6.1. WAP Issuer
Certified WAP issuer shall have sufficient training and knowledge about the hazards
associated with the work or activity, capable of identifying hazards and appropriate
control measures.

The Issuer shall:

6.1.1. Verify the validity of receiver’s certificate and ensure that the receiver
belongs to the same segment he is working for.
6.1.2. Jointly with the receiver conduct the joint site inspection to examine the
work location, identify hazards and set precautions before opening WAP.
6.1.3. Conduct the hazard analysis and ensure all hazards associated with the
proposed job have been identified and properly assessed before each task
using the hazard analysis checklist
6.1.4. Ensure gas testing is performed and all readings are logged on the WAP form
before issuing and after closing WAP. Gas test will be required as deemed
necessary in addition to the fixed gas sensors on locations
Note: SAWCOD sites are equipped with H2S and LEL gas detection system providing a
continuous monitoring during the entire operation (Refer to SIM#2.006 “SAWCOD Toxic
and Combustible Gas Detection System Program”).
6.1.5. Prioritize works and ensure all activities that would create risks if
undertaken at the same time on the same location are suspended until the
priority job is done
6.1.6. Ensure WAP form includes all details, type of Job and tasks to be completed
and acknowledged for the issuance and closure. Also, active WAPs are
displayed clearly on board and accessible to site personnel and auditors for
verification and audit purposes
6.1.7. Ensure all equipment and lines are completely bled off and flushed with
fresh water and ensure no trapped H2S or any other combustible gases
before issuing line braking task or rigging down.

6.1.8. Ensure that all equipment used are certified and or inspected by competent
person or third party and all related documents are available for
verification
6.1.9. Verify the competency of the crew performing the job and ensure their
understanding and awareness of all identified hazards and required control
measures as well as reporting any newly developed hazards for the proper
risk control
6.1.10. Ensure all active WAPs are properly and clearly turned over to the oncoming
issuer and receiver

6.2. WAP Receivers


The Receiver shall:

6.2.1 Ensure he had joint site inspection with the Issuer and WAP is issued
for him before starting any activity
6.2.2 Ensure all employees working under his segment are well trained on all
job related tasks and safety required courses as well as being aware
and understanding the specific hazards, associated risks and required
precautions to complete their job safely and efficiently
6.2.3 Acknowledge the WAP and understand the consequences of non-
compliance with the WAP requirements applicable to the areas where
he is responsible
6.2.4 review with his crew members the task related documentations e.g.
Job Safety Analysis (JSA), prior to the WAP initiation and ensure they
fully understand their responsibilities toward WAP
6.2.5 continually monitor the work and ensure all safety conditions are well
understood and correctly implemented
6.2.6 update the issuer with the task completion or any developed condition
that require his attention
6.2.7 Receiver shall ensure the physical health of equipment used to perform
the tasks as well as valid inspection and PM. He shall report up normal
conditions e.g. equipment failure, break down or multifunction

6.2.8 Ensure all equipment and lines are completely bled off and flushed
with fresh water and ensure no trapped H2S or any other combustible
gases before receiving and acknowledging line braking task or rigging
down
6.2.9 stay on location attending all assigned task till all tasks are completed,
closed and acknowledged
Note: any alteration to the WAP form by the receiver will cause its cancellation and
might result in a certification revoke

6.3. Division Head


Endorsement of division head is required to perform critical jobs with elevated risk(s)
and certain atmospheric condition operations that requires management decision
such as working in gaseous atmosphere at 10-100 ppm H2S , 5 – 10 % LEL.

6.4. OE&C Group


The OE&C Group shall;

6.4.1 Conduct WAP sessions, assessments and administer certification and


recertification.
6.4.2 Issue certificates to SAWCOD employees and service companies personnel who
successfully pass the assessment.
6.4.3 Audit the implementation of the WAP during field visits to ensure discrepancies
are identified and addressed to eliminate future recurrences and share the
report with SAWCOD management.
6.4.4 Perform refresher courses for issuers/receivers who committed errors or
violations in implementing this program.
6.4.5 Revoke WAP ID’s if the issuers or receivers successively violate safety roles
during WAP audits and assessment, demonstrates poor understanding of the
system or deliberate violations that could exposed personnel, equipment, or
production to risk.

6.5. Work Crew


6.5.1 All work crew involved in the activities should have the knowledge and
understanding of WAP applicable to the areas at their responsibilities. They
also shall apply all conditions and precautions specified in the WAP for work
in which they are involved in and to be aware of the non-compliance
consequences.
6.5.2 They should be able to demonstrate a good understanding of the WAP
operated in specified location which they are engaged with.
6.5.3 If in any doubt, or if any circumstances or conditions change, work crew
member (s) shall stop the work, make the work area safe and advice the
issuer immediately
6.6. Gas Tester
Gas Testing shall be carried out by a qualified gas tester utilizing calibrated and
function tested instrument (MX4). Gas testing is required before joint site inspection,
before each rigging up, after rigging down and during line breaking as a condition to
open and close WAP.

Gas tester to continuously monitor toxic and combustible gases as a mandatory task
when a job is performed inside the confined space (well head cellar) as follow;

A- Gas testing to be conducted using MX4 before and during the installation of
the fixed gas detection system inside the cellar. Results to be logged every
15 minutes and after breaks

B- After installing and function testing fixed sensors, continues gas monitoring
will be automatically recorded by the fixed sensors and systematically logged
every 5 seconds. An electronic panel is being installed in the foreman office
and equipped with fault/malfunction alarm as well as display screen for all
sensors readings. The record is being retrieved weekly or as deemed
necessary

Note: If the WAP form gets full in gas test section, then, use the supplemental gas testing
form provided in appendix 17.4

6.7. Confined Space Standby man:


The Confined Space Standby man shall;

6.7.1 Review the confined space entry checklist and related WAP to understand
the confined space hazards, precautions and responsibilities.
6.7.2 Understand the effects of exposure to potential hazardous substance(s) in
the confined space.
6.7.3 Maintain a confined space entry log and perform continuous count of
entrants.
6.7.4 Prevent unauthorized personnel from entering the confined space.
6.7.5 Monitor activities inside and outside the confined space to determine if it is
safe for entrants to enter and/or remain inside the space.
6.7.6 Maintain two-ways communication with entrants to monitor entrant’s status
(e.g., behavioral effects of hazard exposure) and alert entrants of a need to
evacuate the confined space.
6.7.7 Ensure communications equipment are readily available on-site and
immediately notify person in charged (Foremen, supervisor) in event of an
emergency.
6.7.8 Remain at the confined space entry point until relieved by another
designated standby man or until all entrants have exited the confined space.
6.7.9 Never attempt to enter the confined space, even in an emergency.
6.7.10 Never perform any other duties that could interfere with his primary
responsibilities as a confined space standby man.
6.7.11 Order entrants to evacuate the confined space under any of the following
circumstances;
6.7.11.1 An unsafe condition develops inside or outside the confined space.
6.7.11.2 An entrant displays abnormal behavioral effects of hazard exposure.
6.7.11.3 If stand by man must leave for any circumstances without
replacement.
6.7.11.4 During emergency.

7- Works That Require WAP


SAWCOD have identified that all jobs to be performed on gas, oil and water well
locations are hot and require hot WAP. Below tabulated jobs are identified to be
controlled by WAP when job is performed by contractor or any other segment at the
well locations. Tasks within the same WAP are not allowed to overlap each other and
the task shall not start unless the previous one is completed, closed and
acknowledged.

Repeated tasks e.g. multiple slick line runs can be in one task if they are not interrupted
by any other tasks. But, if they are interrupted by any other task e.g. rigging down,

For stimulation operations: each stage will be issued, acknowledged and closed in a
separate WAP.

8- WAP Form and Types


SAWCOD has a single WAP form consists of two (2) copies in two (2) different colors
and controlled by serial number. WAP Form contains the information and tables that
required to be filled and signed off for a specific job on a specific location to meet
safe operation expectations in controlling work hazards.

8.1. WAP Copies

8.1.1 White Copy, is the original copy which shall be kept at the issuer position
posted on the site board
8.1.2 Pink Copy, is the carbon copy which shall be kept at the receiver position
during the job execution

8.2. WAP Types

Although SAWCOD does not have any welding, cutting, or grinding activities, SAWCOD
classifies all of its activities as hot work due to possible hydrocarbon release with
the presence of ignitions that could release from the equipment or machines used to
run the operation. SAWCOD also, does not have variety of confined spaces, but only
gas well cellar that is unified on all locations and have the same design and conditions
however, SAWCOD classified its job types into two categories.

8.2.1. Hot Work


The purpose of the Hot Work WAP is to control work activities that may
produce sufficient energy to ignite flammable atmospheres or combustible
materials. (All operations on well location shall be considered as hot and
required Hot WAP.

8.2.2. Confined Space Work


According to the confined space definition, SAWCOD has the well cellar as a
confined space. Frac tanks used by service companies are classified as

8.2.2.1. Proper preparation and planning of confined space entry.
8.2.2.2. Utilizing confined space checklist Appendix17.5 with confined
space entry log Appendix 17.6 and attaching them to the WAP

8.2.2.3. Safe entry by personnel, including provisions for rescue.
8.2.2.4. During the Confined Space Entry inside well cellars or frac tanks,
aside from personal H2S alarm monitor, continuous gas testing
and monitoring shall be performed by a qualified gas tester.
8.2.2.5. If gas release is expected, the entrants will continuously wear
SCBA or SABA inside the confined space.
8.2.2.6. The personnel entering confined space shall wear approved full-
body harness equipment intended for rescue purposes.

8.2.3. Line Breaking Tasks

8.2.3.1. During the initial opening/line break of any equipment or piping,


the use of respiratory protection equipment (i.e., SCBA or SABA) is
required according to the table in section 8.2.3.10.
8.2.3.2. Continuous use of respiratory protection equipment shall be based
on subsequent atmospheric gas testing per GI 2.709 and this program.
8.2.3.3. Use of the “buddy system” if atmospheric testing indicates H2S
levels at or above 10 ppm. A “buddy system” involves organizing
personnel into work groups so that each individual is designated to
be observed by at least one other individual (e.g., in line-of-sight
with each other).
8.2.3.4. Remove all ignition sources within 23 m (75 ft.) of the point of
opening during the initial opening/line break of any equipment or
piping in flammable liquid or gas service
8.2.3.5. All WAPs adjacent to the area where the equipment opening/line
break will take place, shall be suspended so that no
flammable/combustible material or ignition source is present.
8.2.3.6. No activities to be performed downwind of the equipment
opening/line break when there is a potential release of flammable
or toxic materials.

8.2.3.7. WAP Receiver shall be present to ensure all precautions are in place,
supervise and monitor the line breaking activities.
8.2.3.8. Use appropriate personal protective equipment for the potential
hazard(s) associated with any hazardous material contained within
the equipment/pipe. Refer to the Chemical Hazard Bulletin (or
Material Safety Data Sheet).
8.2.3.9. Atmospheric gas testing shall be conducted before and after the
initial opening/line break of any equipment or piping to verify
acceptable atmospheric conditions before continuing work activities.
Personnel shall continue to use appropriate respiratory protection
until the atmospheric conditions are within acceptable limit.
8.2.3.10. Line Braking Types and requirements

All SAWCOD activities and tasks are including negative line breaking and or
equipment disconnection. However, SAWCOD is using high precautions and
utilizing strong procedures to ensure the safe and successful operation. These
procedures include Wire Line Manual, Coiled Tubing Manual, pumping manual,
SOPs, JSAs and pre job checks as well as the fixed gas detection system in place.

Although negative line break after depressurization and flushing with fresh
water, SAWCOD has classified some line breaks as critical and requires SCBA
donning while breaking as an extra precaution as tabulated below.

9- WAP Issuance and Approval Procedure


9.1. Before the activity commence, the WAP shall be issue to a certified receiver
after joint site inspection completed.
9.2. Joint site inspection shall be conducted by both Issuer and Receiver using
Hazard analysis checklist on the back of the WAP form
9.3. Identify all hazards, applicable precautions and restrictions in the Hazard
Analysis Checklist (at the back of the WAP Form) before initiating each task.
9.4. The Issuer and Receiver must affix their signatures on the completed form.
9.5. The Issuer and Receiver shall acknowledge WAP task hazards & controls in the
acknowledgment table for the tasks.
9.6. Shift handover shall be signed by both receivers and acknowledged by issuer in
the WAP Handover form marked Yes or No on the column provided. (WAP Issuer
shall follow SAWCOD SMS Process 6. 6 Shift Turn Over Process)
9.7. During an Emergency the Issuer may be allow to work without a WAP as long as
work activity does not violate Saudi Aramco minimum safety standards
9.8. If the Receiver must leave the job site, he shall, if the Issuer concurs, resign
supervisory control to another certified receiver until he returns. The alternate
certified Receiver’s name and badge number must be confirmed on the WAP by
the Issuer.

9.9. If by any circumstances the Issuer has to leave, he must hand-over the WAP(s)
to the next SAWCOD employee in charge whom certified as Issuer using SAWCOD
shift turnover form.
9.10. When a line breaking task is required, Section 6 of the WAP form will be filled
by both Issuer and Receiver.
9.11. Gas testing results should be recorded before and after the line breaking in
Section 4 of the WAP form.
9.12. For any reason WAP receiver and crew leaves well location, the WAP shall be
closed, while equipment to be secured, isolated and de-energized.
BPV and Support Unit who perform jobs on location to support SAWCOD site operation
shall follow WAP.

10- Stop Work Authority and WAP cancellation


10.1. The Issuer shall suspend the job and withdraw a WAP if the safety of the job
does not meet with conditions specified on the work authorization. When
conditions are safe again, the Issuer may resume the operation.
10.2. If the contractor commit a violation to the WAP and or any other Saudi Aramco
safety policies, WAP shall be withdrawn and entire job shall be put on hold until
full compliance to all safety policies and guidelines.
10.3. When work is stopped due to any WAP violation and an issue can’t be controlled,
WAP shall be cancelled and reason for cancellation, time and other relevant
information should be written on the WAP form.
10.4. The Receiver has the responsibility to stop work and advise the Issuer of his
action any time if safety requirements and conditions specified on the WAP are
not met.
10.5. If WAP is altered by receiver without the knowledge/concurrence of Issuer, then
it will be considered invalid and shall be immediately cancelled.

11- Closing and Archiving WAP Documents


11.1 Completed WAP’s shall be closed by signatures of both by Issuer and Receiver
11.2 After closing WAPs, white copy and WAP related supplemental documents will be
kept in record for a minimum of three months

12- Receiver’s Shift Handover


For activities that continue for more than one shift, WAP will be handover from one shift to
the other as follow using

12.1 The Receiver will turn over his activity to the incoming Receiver and both will
sign the handover form and acknowledged by issuer. Handover form will become
a part of the WAP and will be attached to the WAP until the work is completed
12.2 The Issuer will hand over all open WAP’s to the incoming Issuer in site/unit shift
turnover according to SMS Process 6.6 “Shift Turnover Process”
12.3 Watchman name is assigned by issuer and included in the WAP handover form

13- WAP Issuers and Receivers Training and Certification


The certificate of issuer and receiver will be issued for two (2) years validity after
successfully completing all required pre-requisite trainings, attending SAWCOD WAP
classroom and passing certification/recertification exam. Certification and recertification
will be obtained by attending classroom for both issuer and receiver and will be tracked by
SAOO MDD. WAP issuers and receivers must have in their possessions valid issuers and
receivers certificates at the time of issuing or receiving WAP. WAP Issuer can delegate the
gas testing task to another qualified gas tester under his direct supervision.

Note: WAP certificate is designed to have the receiver segment reflected on the card. Any
receiver shall not receive a WAP for any other segments even if they belong to the same
company. Receiver who transferred to another company and works under similar segment
and has valid WAP ID shall handed his WAP ID to SAWCOD OE&CG to reflect the changes.

If a receiver is transferred to another service company but under different segment, he


shall wait for at least one year to be certified to ensure full understanding of all job
procedures, hazards and equipment safety.

14- WAP Implementation Auditing

14.1 SAWCOD OE&CG and LP will audit WAP implementation during all field visits,
QSI’s or as needed.
14.2 SAWCOD OE&CG will revoke IDs from receiver or issuer who demonstrates
noncompliance of WAP implementation and committed discrepancies that could
result in or lead to an incident.
14.3 All WAP discrepancies will be reported to the Division Heads for their corrective
actions.

15- Periodic Reviews
