DECLARATION BY PARENT/GUARDIAN OF CANDIDATE

is a candidate for Regular Commission into the Nigerian Armed Forces, do solemnly and sincerely
declare as follows:
1. That I am a citizen of the Federal Republic of Nigeria and hail from _____________________ Local Govt.

2. I agree that my son/daughter/ward, upon invitation by the Nigerian Defence Academy, shall attend the
Armed Forces Selection Board interview.
3. I agree that my son’s/daughter’s/ward’s choice of Department/Faculty as well as Service may be
changed based on identified competencies during the selection process and the needs of the Services.
4. I shall not claim any compensation or relief for any injury or death, which may occur in the course of
tests/exercises conducted by the said Armed Forces Selection Board.
5. My son/daughter/ward and I shall not enter into any correspondence with the Nigerian Defence
Academy on the outcome of the result of the Armed Forces Selection Board.
6. I wholly and knowingly consent that my son/daughter/ward SHOULD join the Armed Forces of Nigeria.
7. I shall not interfere with the training of my son/daughter/ward in whatsoever manner while he or she
is undergoing training in the Nigerian Defence Academy.
8. I shall not claim any compensation or relief for any injury or death which may occur in the course of
my son/daughter/ward’s training in the Nigerian Defence Academy, and subsequently on successful
completion of training, when he or she is in service as a commissioned officer of the Armed Forces of
Nigeria.
9. I shall be responsible for defraying the cost incurred in the training of my son/daughter/ward if he/she
deserts the Academy at any stage of the training.
10. I understand that my son/daughter/ward will be subjected to the provisions of the Armed Forces Act
as amended and he/she will be prosecuted and will not be entitled to any transcript or certificate, as the
case may be, if he or she deserts the Academy or is withdrawn/dismissed on disciplinary grounds.
11. I understand that Examination Malpractice is a criminal offence for which my son/daughter/ward
will be prosecuted in a court of law if indicted.
12. I MAKE THIS SOLEMN DECLARATION CONSCIENTIOUSLY BELIEVING IT TO BE TRUE, in accordance
with the Oaths Act of 1963
