JOINT AFFIDAVIT OF CHANGE COLOR

That We, LUCIANO P. CAONG, hereinafter referred to as FIRST AFFIANT, and ALEX P. GELICAÑA,
hereinafter referred to as SECOND AFFIANT, both of legal age, Filipino, and resident of Matina Pangi,
Davao City, after having been duly sworn to in accordance with law: hereby depose and say:

1. That the FIRST AFFIANT is the registered owner of a certain motor vehicle
particularly described as follows:

2. That the SECOND AFFIANT is the auto painter by occupation with shop located at

3. That the FIRST AFFIANT is desire to change color of the motor vehicle from Black to
Black/YELLOW and the SECOND AFFIANT would undertake the job;

4. That the change color caused to be made nor for the purpose of evading traffic rules
and regulations but for the taste and comfort of the FIRST AFFIANT;

5. That we are executing this affidavit in compliance to the requirements as required by


the authority’s concerned.
