FAULTS IN SCAFFOLD

FOUNDATION FAULTS (SILLS, BASE PLATE, CASTOR)

Top horizontal tube missing

Bottom horizontal tube missing

Girder clamp or beam clamp missing

Top or bottom horizontal tube installed with single clamp

POST FAULTS (STANDARS)

Top checked coupler missing or bottom

Tap or bottom checked couples not fully tight

Trapeze tube installed less or more than 2 feet below runner

Trapeze tube length more than or less than 2 feet

Top or bottom hanger tube installed with swivel tube

Hanger tube installed with double clamp load against bolt Trapeze tube installed with swivel clamp

RUNNERS FAULTS LEDGERS)

Checked coupler below runner

Runner installed with double coupler load against bolt

Overhang less than 4”

BEARERS FAULTS (TRANSOM)

Bearer installed with single clamp butterfly clamp

Board bearer installed with single clamp or butterfly clamp

Board bearer missing

Bearer extended less than 4 feet from runner

BRACING FAULTS

Plan brace installed with swivel clamp


Plan brace installed with double clamp load against bolt

Plan brace installed post to runner or bearer or upright with swivel clamp

Double clamp missing on plan brace

Checked coupler on vertical brace

OUTRIGGERS FAULTS (TIES, RAKERS)

incomplete bas tie

Box tie installed with single clamp or swivel clamp

Butt tube installed with double coupler load against bolt

GUARDRAILS FAULTS

Guard rail install with swivel or single clamp

Guard rail installed with double clamp load against bolt

Guard rail installed below longitudinal rail

Transverse rail installed below longitudinal rail

Upright installed with swivel clamp

Drop bar or self closing bar missing or fixed Top or bottom rail missing

PLATFORM/TOE BOARDS FAULTS

Toe board not secured

Toe board missing

Toe board short and obstructed the access

Plank not secured

Plank overhang less than 6 inches and more than 12 inches

Plank gap more than I inches

Plank split more than 12 inches

Damaged plank used

Toe board installed outside post

Plank secured with nylon rope

LADDER/ACCESS FAULTS
Ladder top or bottom or mid support missing

Ladder not secure at bottom or mid or top

Ladder secure with single or butterfly clamp

Ladder installed incorrect ratio

Access opening less than 27 inches or more than 30 inches

In Front of ladder tripping hazard

CORRECTIVE ACTION

a) provide top horizontal tube

b) provide bottom horizontal tube

c) provide pair of girder or beam clamp

d) replace with girder damp

a) provide checked coupler top or bottom of hanger tube

b) top or bottom checked coupler should be fully tight

c) install trapeze tube shall be approximately 2 feet below runner

) trapeze length nyst be 1.5 to 2 feet

e) replace with double clamp

reinstall double clamp load against hinge flap

replace with double coupler

a) provide checked coupler below runner


b replace with double coupler

a) replace with double coupler

b) replace with double coupler

c) provide board bearer

d) extend atleast 4 inches from runner

a) replace with double coupler

b) reinstall double coupler load against hinge flap c) install Blan brace post to post with double coupler

d) provide double coupler on plan bracing el provide checked coupler on vertical brace

a) provide complete bos tie

b) replace with double coupler

c) install double coupler load against hinge flap

a) replace with double clamp

b) reinstall double clam load against hinge flap

c) install inside post

d) install top o longitudinal rail

e) replace with double clamp

f) provide drop bar and drop bar must be movable

8) provide top or bottom rail
