Greetings!

As part of the K-12 curriculum, the Grade 11 students are set to engage in Practical Research
1 which involves interviewing participants for their study. The study is not only limited to the
conduct of study inside the school premises, but the student researchers may also widen their
research scope to the broader community given that some of their chosen respondents are
outside school premises. With this, please be informed that your child, along with his/ her
research group, has chosen to conduct their study,

requiring data gathering at University of Cebu-Banilad from University of Cebu

The student researchers personally chose the study to provide more novel research and
establish more in-depth knowledge on the present problem they intend to address.
Although, pre-approval of their chosen study, evaluation on the accessibility,
comfortability, and safety of the student researchers had been made, and student
researchers were also encouraged to do their interview online. Furthermore, they made it
clear that they have contact persons in the location they chose and made sure that they
have their guardians with them to ensure their safety.

Granting, the student researchers are only allowed to reach their research setting in small
groups and only during the daylight and go to their chosen location at their own risk. They are
also reminded that their whereabouts must be constantly documented and they are to conduct
their research with utmost care and vigilance.

research endeavors regarding the place and the inclusive dates.
2. I/We hereby freely, knowingly, and voluntarily give my consent to the participation of my son/
daughter in this activity. Attached herewith is a photocopy of my ID for verification and evidence.
3. I/ We acknowledge the possible risk involved but I understand that the school and teachers will do
their best to ensure due care and safety among their students and that the school may not be held
responsible for any untoward incident that may happen beyond their control.
