Dear Madam,
Greetings of peace and love!
I am writing to bring to your attention an unfortunate incident involving one of our teachers. The
details of the incident are as follows:
a. March 21, 2024, around 11:00 in the morning, a teacher from Clemente Cristobal National
High School got into an accident after bringing back the NAT-12 Test materials to the
SDO1Pangasinan Office. The teacher's name is Ed Raymon Q. Caballero, and he's a
Teacher III and a School Testing Coordinator of the said school. He's in charge of handling
test materials for the school and has to take them back to the division office after the exams
are done.
b. According to what he said, he was driving his motorcycle in the Binmaley, Pangasinan area
when big truck suddenly blocked the road. This made him quickly change direction, causing
the vehicle to flip over and resulting the accident.
c. He made it home, but as soon as he got there, he was quickly taken to Blessed Family
Hospital in San Carlos City because his left abdomen was hurt. The medical staff at
Blessed Hospital confirmed that he had minor injuries, including bruises and blood clotting.
He needs to stay in the hospital for a few days for observation. Luckily, there were no other
people in the vehicle with him when the accident happened and no other vehicles were
involved.
I wanted to ensure that you are aware of this situation, as it may have implications for Sir Ed's
ability to fulfill his responsibilities in the immediate future. We are exploring ways to provide
support and assistance to him during his recovery period and to ensure continuity in the delivery of
instruction for his classes.
I kindly request your guidance on any necessary steps we should take to support Sir Ed during
this challenging time and to address any potential impact on the school's operations. If there are
any protocols or procedures, we should follow regarding incidents involving teachers, please
advise us accordingly.
