Dear Ma’am:

We would like to inform your office that we hereby allow our son/daughter Marian Cristine
M. Custodio. to participate in the On – the –Job Training Program required for the degree Bachelor
of Science in Accountancy. We understand that the said program requires 600-hour completion and
will be conducted in _Shore360_ at _ Unit 2F-1A, 2, 3, 4 Philexcel Business Center 6, Philexcel
Business Park M.A Roxas Highway, Zone, Clark Freeport, Clark, 2023 Pampanga_ from _January 22,
2024_ to _May 2024 .

We are aware that the OJT Coordinator will visit and monitor the performance of the
student, and that the immediate superior within the partner institution will take all the necessary
precautions to keep the student safe.

However, if despite all efforts taken, untoward incidents beyond the coordinator’s and
superior’s control occur, we will not hold liable Mabalacat City College nor its partner institution
where our son/daughter is deployed.
