Please explain in writing within five (5) calendar days from receipt of this Show Cause Memo with Return-to-Work Order
why no disciplinary action, including dismissal should be taken against you for the following incidents:
1. Persistent Failure to Notify – Absence Without Leave (AWOL)
Based on our records, you have committed three (3) consecutive Failure to Notify Absences for the following
dates:

Your last reported date was on December 08, 2023. Despite the earnest efforts exhausted by your
supervisor/manager to contact you with your most updated contact number on record, you failed to respond.
In fact, as of this writing, no response has been received from your end, and instead, you have persisted to be
on failure to notify without complying with our Absentee Call-In Procedure.
2. Attendance Policy
Further to this, upon validation, your accumulated unexplained absences which translates to your attendance
infraction has reached a total of 24 Attendance Occurrence Points which is beyond the allowable Eight Points
(8.00) ceiling as mandated by our Attendance Policy.
The above stated incidents maybe considered a violation of any or all of the following company policies and procedures:

Item Number 28. Insubordination including refusal to follow legitimate work orders and instructions of a department
supervisor or other authorized member of management.

Serious Misconduct, Gross and Habitual Neglect of Duties and Willful Disobedience of the Lawful Orders of the Employer.
