Topic Title: Filling out Forms

Hello learners!

Do you have any experiences where a member of your family asked your help to fill-out forms
like biodata? Maybe some of you would say “Yes,” and others would say “No.” Today, you are going
to learn how to fill out forms accurately and efficiently.

What is Biodata?

A biographical (biodata) is detailed information about oneself. A person who is filling out this
kind of form must write information correctly, accurately, and honestly. Biodata is used when
applying or seeking for a job.

It includes relevant information, such as:

1. The personal information-includes general information that helps the recipient understand
who you are (e.g., name, date of birth, gender, height, status, etc.),
2. Educational background-this is where you list in chronological order your academic
achievements stating the place/school and the year graduated.
3. Employment record/history- list of occupational history in chronological order.
4. Character references- includes persons to be contacted to attests to your and interpersonal
abilities, such as your friends and relatives.

These are the things to consider when filling-out a biodata:

1. Read each item in the form very carefully.


2. Follow printed directions.
3. Be precise in supplying the information.
4. Give true and correct information.
5. Avoid making erasures.
6. Handle the form with utmost care.
