S. Lanon, to obtain the birth certificate of my son, Nestor T. Lanon
Jr., on my behalf. This authorization is necessary as I am unable to
personally visit your office at this time.

Please provide Fernan S. Lanon with the necessary


assistance and information required to obtain the birth certificate.
He will present any identification or documents necessary to verify
his identity and relationship to the child.

I understand that by granting this authorization, I am


entrusting Fernan S. Lanon with the responsibility of obtaining the
birth certificate on my behalf. I will not hold the Philippine Statistics
Authority or any of its staff liable for any actions or decisions made
by Fernan S. Lanon while acting on my behalf.
