Note: 1. Application of activities must be approved as follows:


A. Within the school Campus and Koronadal City - 1 week before activity
B. Within Region Xi and XII - 1 Month before the activity
C. Outside Region XI and XII - 2 Months before the activity
D. Outside Mindanao - 6 Months before the activity
2. In case of outside activity, Parents permit of students duly signed by the parents should be attached in
the application form.
3. In case activity needs school facilities/equipment, a request letter address to the Property Custodian
should be attached in the application form.
4. Proposed, budget, program and certificate should be attached in the application form.
5. Adviser/Coordinator must be present during the activity.
6. Submit the activity evaluation form, 15 days after the activity.
7. Accomplishment report/ documentation report should be submitted the most of 1 month after the
activity/program.
