FIELD ATTACHMENT REPORT WRITING FORMAT

INSTRUCTIONS

1. This is the approved field attachment report format for all Maseno University students
proceeding on industrial/ field attachment.
2. Study carefully and seek clarification from University Supervisors.

The field report should be written in correct English (grammar, spellings and punctuations)
and at least contain the following:

SECTION ONE –PRELIMINARY SECTION (NB: pagination in this section is in roman


numerals)

PAGE I- TITLE PAGE OF THE REPORT (Title, Student; name, registration number, course,
year of study and field attachment period).

PAGE II – DECLARATION/ APPROVAL PAGE (student declares his/ her original work and
supervisor approves).

Sample of declaration page

I declare that this attachment report is my original work and has not been presented elsewhere
for the award of any degree.

PAGE III - SUMMARY/ABSTRACT (Not more than ½ page) (NB: should provide summary
of the most important practical work the student learnt, challenges, conclusions and
recommendations)

PAGE IV- ACKNOWLEDGEMENT (the student should thank all those who assisted in/
contributed to the success of the field attachment and report writing)

PAGE V- DEDICATION (Optional)

PAGE VI- TABLE OF CONTENTS (report content arranged in topics and sub-topics, list of;
tables, figures, plates and definition of terms/ abbreviations)
SECTION TWO- MAIN BODY OF THE REPORT (N/B: pagination in this section should
be Arabic numerals)

CHAPTER ONE- INTRODUCTION (one page maximum)

 Background of the field attachment program


 Objectives of the attachment

CHAPTER TWO- PROFILE OF THE ORGANISATION/ DISTRICT ATTACHED (5


pages maximum)

 Background of the organization/ district attached (geographical location, ecological


factors, core business, achievements, challenges etc.)
 Structure and function of the station/ district with emphasis on the department where the
student is deployed

CHAPTER THREE- FIELD ATTACHMENT ACTIVITIES (5 pages maximum)

 Duties and responsibilities


 New knowledge and skills gained in each of the duties and responsibilities assigned
 Thing that interested you most and why and things that interested you least and why
 Challenges experienced and how they were handed

CHAPTER FOUR- INNOVATIVE PROJECT (2 pages maximum)

 Objective, Methodology, Results/Outcome

CHAPTER FIVE- CONCLUSION AND RECOMMENDATION (2 pages maximum)

 Conclusions (remember to include the major strengths and weakness of the field
attachment program
 Recommendations- include recommendation for strengthening the field attachment
program

REFERENCES

APPENDICES- include any other annexes to the report such as drawings, charts, maps,
photographs etc

NB: Report MUST be typed, bound properly and written in Times New Roman font Style. The
font size should be 12 and 1.5 line spacing.
