2. Passports-biopage (copy of most recent & old passports including pages with stamps)
3. Digital Photo (passport size, white background)
4. Proof of Medical certificate - fit to travel
5. Booking certificate - you may buy at a ticketing agency (will cost around Php500- Php1k)
6. Police certificates for all countries where you have stayed for 6 months or more
(Note: For Philippines, NBI certificate is valid for 3 months only)

Funds (should cover airfare + living expenses): minimum of Php300k per applicant
 IF they need financial Support
 Affidavit of Support/ Letter of Support/ Statutory Declaration
 Family Tree - establish relationship
 Birth Certificate / Marriage Certificate of all supporters involved
 Identification Card (driver's license, voter's ID, passport, visa, citizenship ID) of supporters
 IF supporter is employed
 Proof of Work: COE, contracts, seaman's book, shipping history, T4, ITR, payslip for the past
3 months & any other relevant documents
 Proof of retirement: certificate of retirement/pension, previous service record
 IF supporter has business
 Proof of Business: Mayor's Permit, BIR Certificate of Registration, SEC, Income Tax Return
(latest), Auditor's Report
 Proof of Property: OR/CR Vehicle with Insurance details, Land Titles with Tax Declaration,
Certificate of Ownership
 Submit the following
 Proof of Liquid funds: Bank certificate with past 6 months SOA, Stocks, Insurances with fund
value, bonds, time deposits
 Proof of Billings past 3 months
11. Supporting Documents from Canada
 Invitation letter
 Proof of Status in Canada (PR Card, canadian citizenship card/certificate)
 Birth Certificate / Marriage Certificate of all supporters involve
 Proof of billing - past 3 months with Canada address
 Proof of work, T4, payslip
 Proof of funds - bank statements, properties, bonds
