Nabielah Panolong Macadato filed an affidavit of loss stating that she lost her Globe prepaid SIM card number 0966-952-6737. She used two SIM cards, one for Globe and one for Smart. On July 13, 2022, she removed the Globe SIM from her phone and placed it in her wallet. When she got home, she noticed the Globe SIM was no longer in her wallet. Despite searching, she was unable to find the lost Globe SIM. She executed this affidavit to support her application for re-issuance of the Globe SIM number.

AFFIDAVIT OF LOSS

I, NABIELAH PANOLONG MACADATO, of legal age, Filipino, married


and a resident of Bangon. Marawi City, after being duly sworn in accordance
with law, depose and state:

1. That I am a regular user of a Globe Mobile Prepaid Card with No.


0966-952-6737;

2. That I used two Sims in my cell phone one for globe and other was for
smart;

3. That sometime on 13 July 2022, when I went to Marawi City the said
globe sim was removed from my cell phone and placed it in my wallet,
when I got home I took the globe sim to put it back in my cell phone
but I noticed that my globe sim was no longer in my wallet where I
placed the same;

4. I exerted all efforts to locate my globe sim but the same could no
longer be found and is now beyond recovery; hence, it is considered
lost for all intents and purposes;

5. I am executing this affidavit to support my application of re-issuance


of my Globe sim number 0966-952-6737, to attest to the truth of the
foregoing facts and for whatever legal purpose this may validly serve.
