AFFIDAVIT OF LOSS

I, _________________, of legal age, single, Filipino and with residence address at


_______________________, Gen. Trias City Cavite, Philippines, after having been sworn
in accordance with law, do hereby depose and say:

1. That I am a regular user of Globe Mobile Subscriber Identity Module (SIM) Card
with Number 0000000000000 since 2018;

2. That the aforesaid SIM Card is currently registered in G-Cash App with cash
balance of Php ____________;

3. That sometime of 25 October 2023, at around 2:00 in the afternoon when I was in
my workplace, I noticed that the aforesaid SIM card kept inside between the case
and cellphone unit was missing;

4. That despite diligent effort to locate the same it was nowhere to be found and is
now beyond recovery;

5. That I am executing this affidavit of loss to attest to the truth of the foregoing and
to support my application for the replacement of the said SIM Card to Globe
Telecom Company and for whatever legal purpose this may serve.
