AFFIDAVIT OF EXPLANATION AND UNDERTAKING

I, ROLANDO E. CENIA, Filipino, of legal age, married and a resident of Panghagban,


Buenavista, Bohol, after being sworn in accordance with law, hereby depose and say:

1. That I am a bonafide holder of Professional Driver’s License No. G03-07-002979 issued by the
Land Transportation Office, Tagbilaran City on October 17, 2007 for which I was issued a Temporary
License – Pro which was valid until March 17, 2008;

2. That until the present, I have not claimed or received my laminated Professional Driver’s
License for which I am now applying for its release;

3. That I was not able to claim the said laminated Professional Driver’s License before the
expiration of the validity period of my temporary license because I was not able to find time to come to
Tagbilaran City due to the distance of my residence and the City;

4. That I hereby undertake to comply with all the requirements of LTO-Tagbilaran for the release
of my said laminated Professional Driver’s License, including, but shall not be limited to, the submission
of a new drug test result and the payment of all the necessary fees and charges thereof;

5. That I execute this affidavit to comply with the requirements of LTO-Tagbilaran City for
release of my laminated Professional Driver’s License;

6. That further, I execute this affidavit for whatever legal purpose this may serve.

IN WITNESS WHEREOF, I hereunto set my hand this 7 th day of October 2009 at Tagbilaran
City, Bohol, Philippines.
