AFFIDAVIT OF CORRECTION

I, _________________________, of legal age, widow, Filipino, and a resident


of Mabini, Batangas, after being sworn to in accordance with law hereby
depose and say:

1. I am the registered owner of the parcel of land identified as Lot No.


___________ located in Mag-Asawang Tubig, Naujan, Oriental Mindoro,
with an area of ONE THOUSAND TWO HUNDRED FORTY SQUARE
METERS (1,240 SQ.M.) covered by Transfer Certificate of Title No.
_________________ issued by Registry of Deeds for Calapan, Mindoro
Oriental, hereinafter referred to as the “PROPERTY”;

2. I purchased the said PROPERTY decades after the date of death of my


husband, _____________________, but for some reasons, the conveyance
documents used at the time I purchased said PROPERTY, mistakenly
failed to indicate my true and correct civil status;

3. That I am executing this affidavit to attest to the truth of the foregoing, to


request the Register of Deeds Calapan, Mindoro Oriental to annotate this
affidavit to reflect that I am a long-time widow prior to the purchase and
registration of conveyance to the PROPERTY, and for whatever legal
purpose it may serve.
